There is a good chance it does not check the actual ragdoll. Easier and less performance loss if use the camera position and other factors. It would not surprise me if it relies on the server code to check for it, but I don’t think so.

There are so many achievements after 5000 hours I still get new ones. March 25 I got party crasher to kill 3 people doing conga. March 11 I got photostroika to provide freeze cam of me taunting while ubered. Those aren't even rare, but if you never hunt for them you just have so many to do.

The reason TF2's skill ceiling is limitless is because it is litterally impossible to unlock all of the achievements.

using the regular rocket launcher was such a bad idea for the parachute achievement lol. he was basically banking on a random crit.
Given that the chance increases with damage done, they are quite common for high-level players.
