Because in evangelion when these “robots” gets damaged, usually the pilot will feel some pain at the place the robot got damaged. In this scene however, asuka was completely connected to the robot, which means that although she can fight like a god off war, the downside is that if the robot gets injured, she actually gets the injury. In the extended clip, you can see her bleeding from her eye. Yes i know that my comment is pretty late but i just wanted to answer your question
