    Congratulations !!! You will be a great member of ANY hospital/clinic !!! I have a feeling, your patients and co-workers will be happy to work with you ! You explain things so well..this will make you a great parent too! lecturing kids WORKS !! (at least for me!) I always wanted an "explanation" for everything. (still do). Tell everyone about colon cancer screening and the importance(we lost close family friends to colon cancer)

    I work in surgery, we have outpatient GI days for one of our OR's and of all the docs I work with, the GI guys are my favorite! So much empathy and patience, always a kind word for those of us who support them.

    Congratulations! I loved living in St. Louis, so I can see why you wanted to stay. There are also some good hospitals (ok, some not so much, too). Back in the 80’s, I was treated very well at Barnes. It must be such a relief to see the light at the end of the tunnel - savor that wonderful feeling!

    I love the typos in his video. I am an attorney in an area of law that I must be very detail-oriented, but outside of work I can be a real space cadet. It goes to show that you can be excellent at an intellectually rigorous job but that you aren't perfect at everything :-P Anyways, congrats to Doc Schmidt! This was very interesting and exciting to hear about.

    My stepdaughter is an ER Surgeon in Virginia, so I find your skits and other Doctor stuff is really entertaining. Actually you've given me some insight to engage her in conversation.
    Topic idea- achieving partner within a group. The insider benefit when receiving medical treatment; from other medical professionals. Ex my father in law is a Dr, Brother-in-law is a Dr, they get a very difficult to schedule appointment with the top Neurosurgeon in the region.

    While I know that $ isn't everything, how much do you think student debt plays a part in the decision process when doctors need to make such decisions?

      Id say it plays a decent role - just check out average pay for the specialties vs how competitive they are. Some specialties can make paying off private med school ($$$) debt hard.

    Congratulations and welcome to STL! I will give you my 3 pearls of wisdom that I have given every new doc for the last 30+ years. 1. Figure out who is in charge. It's not always the person with the title. 2. Always make friends with the ward clerk, housekeeping, and dietary. That way your orders get processed, you always know the latest gossip, and you'll always have food saved back. Finally, 3. Align yourself with a competent nurse. They will keep you from killing your patients! Good luck and many happy years of practice to you

    That was interesting. Missouri is somewhat unusual in that physician non-compete contracts are enforceable. But they must be only sufficiently reasonable to protect the legitimate interests of the practice. In many states, including my own, they are simply not permitted as a matter of law or public policy. You must be careful that your practice does not run afoul of the federal anti-kickback and self-referral laws. I was intimately familiar with the workings of several large subspecialty practices that did not do well and were ultimately sold. Physicians failed to keep track of and minimize overhead. They over-extended themselves financially usually as the result of hiring incompetent managers whose interests were not aligned with those of their employers. Some should have had their operations reviewed on occasion by forensic accountants! I recall one large group which was operating in violation of certain Federal statutes. I warned one of their senior members and he simply said that they had expert advice of legal counsel and they were therefore comfortable. That was fine until a disgruntled employee copied records and went to the Attorney General and that was that. The state didn't revoke their licenses but could have.
    Another thing to be aware of is that your pension funds will be invested somewhere and investment managers are usually very expensive virtually worthless. The investment vehicles are usually grossly over-priced and virtually guaranteed to under-perform simple market averages. IF they seem to be out-performing over time, the figures are being manipulated. I've been around a long time and I've seen a lot. You seem to be a very bright young doc. so keep your eyes open and be careful. Enjoy GI. It was my first fellowship offer.

    Thank you Doc for sharing your expereince! Question, Is it common to hire a lawyer when reviewing contracts? What kind of lawyers would you have to look for specifically? I have family members who are doctors and I've never heard about them hiring lawyers to negotiate contracts. Is this just for private practices or even academic settings?       I think it's fairly common. It's definitely not required though, and it seems to be a younger generation thing- my dad didn't use one when he signed his contract for example. There are lawyers who specifically focus in healthcare or contracts

      Benefit of hiring a lawyer prevents animosity between negotiating parties. It checks the adversarial attitude at the door, instead of nit-picking in the future because one party thinks they lost.

      I am not allowed to solicit them (ie "come see me at this new job instead") but if they find out on their own where I am going, then they are allowed to come see me at the new practice

      yes! Many jobs offer that. However sometimes it is better to just get a larger signing bonus so you have more flexibility (you can still use the signing bonus to pay off loans for example)

    Hearing doctors talk about money makes me think most of them should have probably gone into finance or business since they usually seems to be highly interested in it. And I say that as a professional investor. Hoping Tesla will tackle healthcare once clean energy is figured out and the doctor robots will just show up at your door when needed with the latest/best science downloaded and it will cost pennies.
