  • Miliary tuberculosis is a form of dissiminated or extrapulmonary tuberculosis that is caused by sudden diffuse dissemination of tubercle bacilli through the bloodstream (hematogenous spread of TB).
Miliary Tuberculosis mainly occurs in children and young adults but may also occur in older people and it is insidious in onset in this older age group.
Miliary TB is can be difficult to diagnose especially in older age group in which case it is known as Cryptic Tuberculosis (because of its insidious onset). It occurs when the organisms reach the blood stream, either in primary or secondary tuberculosis. Miliary TB got its name in 1700 from John Jacob Manget based on how it appears on autopsy findings.
The bodies would have a lot of very small spots similar to hundreds of tiny seeds about 2 millimeters long scatted in various tissues.
Since a millet seed is about that size, the condition became known as miliary TB.
It’s a fatal disease if left untreated it would kill the patient in days or few weeks. The miliary lesions may enlarge and coalesce, resulting in consolidation of large regions or even whole lobes of the lung. Systemic miliary tuberculosis occurs when bacteria disseminate through the systemic arterial system. The liver, bone marrow, spleen, adrenals, meninges, kidneys, fallopian tubes, and epididymis, but could involve any organ. 10 to 30% of adults, and 20-40% of children with miliary tuberculosis have tuberculous meningitis.

Question:So if miliary tuberculosis is extrapulmonary tuberculosis as you said at the beginning of the video, why do we see these millet-like lesions in the lungs? Does that mean it is both Pulmonary and extrapulmonary??🤔🤔

Miliary pulmonary disease occurs when organisms draining through lymphatics enter the venous blood and circulate back to the lung. Individual lesions are either microscopic or small, visible (2-mm) foci of yellow-white consolidation scattered through the lung parenchyma (the adjective “miliary” is derived from the resemblance of these foci to millet seeds). Miliary lesions may expand and coalesce, resulting in consolidation of large regions or even whole lobes of the lung.
