The two main forms would be hover form which you may see in the original machine and humanoid form which you see right here now.

Covered extensively by The Grateful Dead; this video version is a cover of their version... You can really hear the Jerry melodies in pinball form....
This game I always saw at this pizza parlor called big Pete’s New York style pizza But it closed to make way for another New York style pizzeria called Paul’s I remember always being startled by the bell inside the machine it was really loud
How did you get such a high quality studio sounding recording? Did you record it from the machine and then EQ it or something? Or do you have access to an old Williams tape of it?

Early machines had the woman's name as Marilyn, it was changed to Lola in later machines.
Because Marilyn Monroe was copyrighted. Once I saw a Taxi machine that actually had both Marilyn on the playfield and Lola on the backglass!
Bought one brand new for my pinball operation here in Norway. Should have kept it ! I am out of the arcade business by now ...

I think he used a VGM player, the sound chip should be standard, maybe a soundblaster or if is a midi he assigned samples to the drums
