They’re not packing in nutritious food or anything but they’re packing enough quantity to fill most needs out of sheer amount. She’s not malnourished, undernourished or anything. Pretty sure her doctor would pick up on any bad deficiencies too.

      Because he ACTUALLY wants these people to live good and long lives! He’s such a good man, not just looking to make money like a lot of other doctors. He wants his patients to be healthy.

    That doctor has probably heard every excuse that exists. He is honest, stays calm, and tells the patients the logical consequences to their behavior. Many of the noncompliant patients must be in denial and think that gastric bypass is a one step cure that magically fixes all of their bad habits. Only those patients who continue to follow the doctor's instructions after surgery are successful at losing weight and keeping it off.

      yep, patients who do their end of their bargain are able to lose weight. The only thing doctors, nurses and PAs can is to say what is necessary without intruding privacy

    Her compulsion to eat is more powerful than anything the doctor has to say... She knew he was right, which I assume that's why she left - the doctor was too close to the truth.

      I am not nearly as big as her, but yeah, that seems to be the reason. I am a littme overweight aswell (not that extreme but still have to loose like 10 or 15 kgs) and my mom is pretty open with me.
      She often tells me that she sees how that extra weighr affects me and that it would be so easy for me if I just had the willpower. And hearing that you did something wrong hurts. Cause you know you eat too much, you eat when you arent hungry, when you're bored or not the right foods. And sometimes, fleeing the conversation is how people deal with it. Even if its not the best way.
      I probably would have started to cry.

    some of these people think surgery is the magic cure and that they can still eat what they want, just not as much, doesn't work that way. You have to commit to a lifestyle change, not just a change in diet. They get mad most times because they know he's right.

    I've never watched these shows so maybe this happens but these people literally have addictions and addicts need serious support to break them. Everyone on this show needs to be in therapy, period.

      Yeah they do get therapy but I wonder if most of them are able to afford continuing therapy for very long. It’s not covered by many insurances so that would be very expensive

      @@sierracase5515 TLC pays any/all medical/travel costs iirc as compensation for being on the show, and thats all they pay.

      @@booosadan I wasn’t assuming tlc pays for the therapy, I just mean maybe the patient can afford one or two that are required for approval, but not in the long term or for enough time to resolve mental issues that have been around for decades

      To be fair, I couldn't tell if they were a man or woman when I saw the episode, but I think I guessed female at the time too. They have no hair, no facial hair/visible arm hair and thin lips, among other things. The high voice doesn't help matters, making them difficult to gender. No hate, just saying. I won't go on a tangent because I don't want to be attacked, but I will say that a lot of men are less masculine than they were 100 years ago. Back then, you didn't have to guess or assume, you knew a man from a woman.

    I kind of feel bad for these people. I know they’re grown ups and it’s their choice to keep their eating habits, but I feel like once you’ve spent your entire life eating that way and being a certain weight, it’s hard to stop. They literally have a food addiction. I feel the U.S. does not do enough to combat the obesity problem we have here. Partially, I think, because they want to keep people sick. It’s just a sad issue. I wish something would be done about it.

      just like you said, its food addiction, so people wont change unless they want to change themselves. The government can do small things like introduce school kids to healthy eating habits (I had this course in Australian primary scool) but these kids have generational issues from their parents and they will always be the greater influence more than a school class.

      @@catmerchant8699 I think it’s too hard to change for many grown ups who grew up surrounded by fast food and unhealthy eating habits, but something needs to change for the younger generations and the government has to push for it. Like you said, a big thing is educating from a young age about healthy eating habits and what can happen to your body when you reach obesity. I feel the government needs to regulate these fast food places and many other food places on what they use to make food, portion sizes and all that. Some of these portions are insane! The U.S. is extremely gluttonous. They want everything big and cheap. Sadly, obesity is also linked to economic struggles so it’s pretty complex. The hard thing is here in the U.S., people don’t like to be told what to do, but I still think always ranking either #1 or #2 for obesity in the world is something politicians should care more about but they don’t :/

      @@nanainadream4874 Most people also simply dont have the willpower or other issues. I need to loose 10 to 15 kgs. My biggest issue is not having the willpower and eating out of boredom. I can controll the latter most of the time but just keeping up with it is pretty hard. Plus the mental health issues I also have.
      But I am only slightly overweight and dont gain at all. I am staying at my weight. So its not as extreme as in this cases. When you are that obese, you need to have the willpower and see how bad it is.

      @@Shyress agreed, and tbh I think the mental health issue is a huge key in a lot of these obese people, like depression, anxiety and just ED’s in general. But the American privatized healthcare system kinda wards off people from accessing the care they need :/

      Did you see what happened when American school systems tried to provide healthier food alternatives to children? The parents lost their damn minds. I cAn FEeD mY KiDS whATeVeR I WaNT. The system tries but there’s no accountability or repercussions for slowly killing your children/family.

    Well I am overweight, I do not look malnourished at all trust me, but when the doctor did the blood work, guess what? Yep my blood showed I was malnourished. I barely eat yet I don't gain, but I don't lose. So I took a good hard look at what little I did eat, dumped bread when to rice added veggies to my diet and cut sugar down to almost nothing. I cook mostly Korean food now but also Chinese and Japanese and guess what? Yep I have lost 8 pound in 1 week alone. SHOCKER! NOT! Americans do not even know what healthy food is anymore cause we are told the processed, prepacked food is "healthy." Nope go get the fresh foods and cook it yourself.

      I think that's a problem with doctors too, as an med student I always hear the and a balance diet, but guess what? Everytime someone ask directly what should I eat? The answer they give is: oh well, healthy food, eat fruits and vegetables, and like ok, that's right, you should eat that, but that's not the frickin right answer, refer the patient to a nutritionist make them make a healthy plan, with a day by day options for you to eat, explicitly forbid some foods, like if you tell someone yeah, you can eat cheese maybe you are thinking they will eat 30gr of cheese and the patient thinks is ok to eat 1kg 🤷 honestly lacking of communication is the problem for a lot of people in this situations

      And what if you don't know how to cook? I don't know how to cook, I was never taught how. Why? My mom's reasoning was that I wasn't interested as a child. Like okay, but even if I wasn't interested, you should have sat me down and taught me anyway. You should have made me sit and pay attention, and since you didn't, I'm 24 and don't have the slightest idea how to cook anything.

I lived with my grandma who always made everything cause she said that I could hurt myself, then when I started to live with my mom I have to learn on my own by necessity cause it was that or starving, at least the basic, after some burns/cuts etc. I learned how to cook by practicing, and looking tutorials, so you just have to do it... yeah, it sucks that people don't teach how to do things so you have to figure it out on your own, but that's what being an adult is, solving your own problems

Some of the secondary sex characteristics that we use to differentiate between men and women become less obvious when there is so much fat covering the body and face. I believe you about the estrogen though.

    When I first watched this show when I was younger, he seemed like he didn't care. Now that I'm older I realized he does care, he just is very realistic and knows he's dealing with food addicts.

    I believe food addiction is one of the more serious addictions. You cant survive without food, theres temptation everywhere, theres misinformation everywhere and people treat you like a child if you have an issue with it. Dr Now only sends them to a therapist once in a while. I think it should be mandatory. Imagine a doctor screaming at you that you're childish and unacceptable because you relapsed on drugs. I both love and hate this show tbh

    Malnourished means nourished badly, which can be either under or over nourished. So yes, that person is definitely malnourished, and probably has a huge lack of essential nutrients

      It cannot and does not mean over nourished, although you can certainly eat a lot of crappy food choices that don't provide much nutrition. However, when you are 600lbs, no matter what you're eating, you'll be getting the essential vitamins and minerals required to survive because of portion size. You might eat crap day in and day out, but even junk has some nutrients...It just also provides too much fat and calories and sugar. Eat enough of the junk and your body will be getting the few nutrients provided and it all adds up...so no, you're not going to be over (is that even a thing, had taking too many of certain supplements? I have doubts you can over nourish via food) or under nourished. But anyways, malnourished means you're starving your body and that is not even possible at 600lbs. Even if they do nothing but drink water for 6 weeks, they will not be malnourished because the body has stored all the nutrients it needs to survive for quite a long time.

      @@MrsBrit1 you can be malnourished/deficient in certain nutrients if the foods you are eating have basically nothing in them to benefit you regardless of how much you weight and how much you eat.

    The second episode you showed I remember really well. I remember wondering what the hell sex they each were? The guy I actually (at first) thought was a woman!!!! People like that guy are very narcissistic. They bully and are violent until they get what they want. Dr. Now (whom I love for his no nonsense approach) has the same old excuses, day in, day out. I think it’s brilliant that he actually calls these people out. A lot of them use him; in that, they tell there family/friends they’re going to get help, when really they don’t want the help at all! Friends and family lose patience with them because they tell the same old excuses over and over and over and they’ve had enough! Often, the food abusers want to keep their slaves, but have no intention of doing what it takes. Dr. Now is the excuse they use to show they are ‘serious’ to their enablers, when they are not serious at all! There are many episodes where Dr. Now has shouted at them AND called their bluff as well. I understand food can be an addiction, but many of them (narcissists) are looking for the easy way out. What they don’t understand is there is no easy way out! Even with surgery. There are only a handful who have gone to this brilliant doctor who actually want his help... they are they ones who are disciplined, determined, and stick with it!

    Had an american cousin living with me for 3 weeks, she lost 20kilos on my homemade Norwegian food.... Reasons:no fried food, no sugar, fish 3 times a week(boiled or grilled) 1 vegetarian day....

      It reminds me how I was called tiny in Canada even though I was overweight at the time (going through a burnout, I started having bad eating habits as coping mecanism) with 56kg for 1.58m. My usual weight is 49.5/50kg. Back in France, I dropped back to 52kg in 4 months, not even because I had changed my eating habits at the time, the stuff I ate just had considerably less sugar in it, and I was back to my "let's walk everywhere" rhythm (in a hilly countryside town) instead of driving everywhere because distances in Canada are so much bigger than in Europe so it's not as easy to just walk your way around wherever you want to go.     For me, the worst episode is the one where the patient kept asking him if he believed in God. She said that if God wanted her to lose weight, then she would lose weight 🤦‍♀️
    And I find it funny how so many of them tell him that they didn't lose weight because they're bloated lmao who do they think they're fooling ughhh it's do frustrating to watch

    His bedside manner needs a lot of work. He's talking emotionally in a professional environment to someone who is very vulnerable. He should explain that she's throwing up because she's overeating. That the surgery was designed so that she couldn't overeat and could lose weight pretty quickly. That he's concerned that she isn't losing weight as expected and then ask her what her average day looks like, and walk her through alternatives and possibly gaining a better understanding of what's going on. This should be a team effort, not a confrontation. When you get aggressive, the other person either shuts down or gets aggressive in response. My primary physician doesn't even respond. I contacted him nearly 2 weeks ago about a bowel obstruction (which could lead to an intestinal explosion and quick death). I was told to wait up to 3 days for reply. It's been nearly 2 weeks so I'm thinking of going to the emergency room, getting treatment and "firing" him. This isn't even the 5th time he's done this in the last 9 months. When doctors don't treat their patients like people with a medical problem and instead treat them like a nuisance, nothing gets accomplished. That's what Dr Now is doing. He's speaking the truth, but he needs to calm down about it and stop using attack language (like "are you serious" and "no you're going to sit back down")

      I also wonder if cost/understaffing is preventing them from seeing a counsellor or psychologist. A lot of countries, including mine, have an absolutely garbage mental healthcare system.

    6:08 I know youre making a joke, and it probably doesnt apply in this particular situation with this patient (I havent watched the full show so can't comment) but actually malnourishment following a bypass is one of the big side effects of any type of bypass. Its quite common to have health complications and some people have even starved to death despite still looking 'fat' following a bypass because their body hasnt been able to absorb enough nutrients to keep them alive.

    It’s always frustrating watching 600-lb life because it seems like the folks are given a relatively small weight loss goal in a short amount and then they struggle to meet it. When they finally do, they get the surgery. But afterwards they struggle to follow through. It would seem that the problem is in the first step: they didn’t have enough time to work hard to make a change and implement a new lifestyle. They seem to really think the surgery will solve everything after they have it. It’s so unfortunate.

    I'm pretty certain that woman is a man, but Dr Now doesn't hold back with anyone! He's like "I'm too old for your crap, and I know more about your issues than you do, so I'm laying down the law!" These people surely know what they're getting with him before going to him for help. He doesn't treat you with kid gloves 24/7 because he wants you to succeed.
