I'm pretty sure that the block pushed by the piston is really actually outside but Minecraft can not load the block outside, so a ghost block is placed.
I haven’t tested this myself but i heard you can go in the nether by the border and walk in the nether for a while then build a new portal and you will come out past the border further than that.

That's an interesting theory. That would explain why it allowed me to boat inside of it without taking damage!

I believe the most you could go past the border is 8 blocks since a block in the nether equals 8 blocks on the surface, but i will try this theory!
