Intercostal neuralgia can be debilitating for patients, often resulting in severe chest pain and decreased quality of life. Cryoneurolysis offers a minimally invasive solution for pain relief by selectively targeting and disrupting the sensory nerves responsible for transmitting pain signals.
Description of the procedure:
The target rib and level is palpated, confirmed with C-arm and marked. Local anesthetic is then infiltrated and then skin is punctured with a large bore needle. High frequency ultrasound probe is then place in the mid scapular line. The rib, pleura, intercostal muscles are identified. Then 18 gauze cryoprobe is advanced just below the inferior edge of the rib. Freezing is then started. As freezing is progressed, an iceball is formed at the tip of the probe. The size of the iceball is approximately 8 mm in length and 6 mm in width. Freezing for 3 minutes is followed by thawing of one minute. Then the cryoprobe is withdrawn.
