It's not necessary to remove the front right wheel, but it is a major pita. Be prepared to scratch your hands and forearms up real good doing this job. I did this on my 2011 Highlander with the same 2.7L motor.
Yes, you don't have to remove tire, but it's harder work the the belt around especially non-saavy weekend mechanics. With floor jack and impact gun, it's quite easy to remove the tire and it'll provide much clear view to entire belt looping around.

Toyota service recommends between 50K to 100K miles. That said, these OEM belt seem to easily last 100K miles. My friend’s Venza belt last over 180K miles, but to me that’s too dangerous to go that far when the belt is only $20-25 bucks and replacement is easily doable at home.

I was also surprised by this method, but is was not better to remove the engine altogether and it would also be more convenient to change the belt!

Do you recommend changing the tensioner too? The car has 107k miles.
Not necessary unless the tension is weak.

@@BadWeatherfreak Too many fake OEMs are out there for auto parts. I would buy it from a Toyota dealership. The belt is not too expensive.

What model and year car do you have, and how many miles do you have? I always try to change my drive belts close to or before 100K miles.
