Thanks Kevin, one small tipp to add: I always start putting the Excel file in a subfolder Questionaires. Otherwise you have book1 book2 etc in your root directory. If you later move the Excel sheet to another directory, the online form is always connected to this Excel file.

Great video Kevin, but what if someone wants to order more than 1 type of cookie? I tried to set up a similar form as a purchase requisition form for my company, but got stuck when trying to allow for an unknown number of items to be requested. Each item will have the same base data (requester, approver, department, etc), but of course different description, part no, quantity etc. Any ideas on how this could be accomplished with MS Forms?
this is very good, but like they have restric to not everyone can use, google is on ahead, on google sheets you have the sheetapp can do it those form, and much more than that. But thank you, is good to know there is another alternatives.

Hi Kevin. This is the first time I have been to your site and you explained one part of what I needed so well, I decided to write and ask you about the other part. I am part of a team and we have a request form for our colleagues to use when they need assistance. When a request comes in, our team receives an alert and we all go in to see on the excel side and someone completes the rest of the spreadsheet with who is going to assist with the work and the status of the work until completion. Recently, someone on our team went in and added the requests directly onto the spreadsheet on behalf of the requester. This caused some errors like the number of the request that is automatically populated and the status section which has conditional formatting in it. In addition, now when a requester adds something to the form at times it does not show up in the excel spreadsheet. I was wondering if there is a way to correct these issues? I thank you for any assistance you can give!
OMG, I loved it, I've already spent the entire day trying to figure out something like this to make it simple for my estimators to upload their projects. How can this be extended to a list of components for a roofing system? The columns are always the same ID, Cost Code, Title, Unit Price and Unit Type but each rood has several lines, one for the membrane, one for the Cover Board, one For the Insulation, one for Fasteners and so on. I don't see how to use this but instead a Fully Automated Data Entry and the coding is driving me crazy.

I have a paid personal Office 365 account and I don't see the Insert > Form option when I try through Excel on the web or on Excel on the desktop. :(
