NASA has always said that "Space is Hard". I suspect that with SpaceX or Blue Origin there will always be tragic moments with missions that actually have a human crew. Occasionally and unfortunately it happens with passenger domestic aircraft and will continue to happen. Consider the many people who die in car accidents yearly.
7:51 T+7:40 first sea level engine failure. 8:13 T+8:02 second sea level engine failure. 8:14 T+8:03 first vacuum raptor failure. 8:29 T+8:18 second vacuum raptor failure. 8:35 T+8:23 third sea level raptor failure, and 8:37 T+8:25 loss of telemetry.
Just an observation. Take a look at the fuel levels before the first engine went out (flight time 7:40). The levels had been decreasing in line with each other. LOX and CH4 started to differ after the engine out, with the Methane levels falling rapidly with each engine faliure. Given the fire we saw in the flap hinge, could there have been a rupture in a line running methane that got worse, taking out multiple engines leading to a RUD?
I noticed this too and wondered if there was a blockage in the LOX side. However the rapid decrease of CH4 lends toward your CH4 line rupture theory better.
That reminded me the intro of the TV show DEBRIS. How sad is to watch this. Hope they can check what happend and fix it. I still have faith in the Starship Project.

When this ship is certified to carry passengers and not a test article, then we can discuss how many would be on a Starship. This is not a finished product. It is a test vehicle that is pushed to its limits to find out what the limits are so the real thing doesn’t get people killed. Even then, getting to space is dangerous. Then again, so is driving a car, or flying a plane.
The early video animations from SpaceX mentioned a crew of 50 in the Starship, but unmanned missions had to deliver tons of equipment to Mars before a crew would go. I presume the first few manned missions to Mars will have a rather small crew; probably less than 10.
This is the 2nd catch of the Booster. The day that there is a "RUD" during the catch attempt will be a Game Changer for SpaceX and would probably end further attempts at catching the Booster. Perhaps Elon could Land the Booster on a Barge-Ship located in the Gulf of America!

The churn of money means jobs to raise families & buy homes, etc., and some to taxes to build schools and American infrastructure. And the metal & fuel of the rocket itself: All of it came out of the ground, and all of it will eventually go back in the ground. So not a waste of money if you see the Big Picture.

Perhaps they should focus more on getting the ship into orbit and less on capturing the booster that will require millions of dollars to refurbish prior to the next flight.

that's nothing... not an issue.. the issue is at 8:05 you can see flames in the aft flap hinge.. looks like there was a fire onboard.

I concur; the door to the pez dispenser was likely insufficently braced against MaxQ, and it caved in, and then the thermals rapidly climbed.
