  • Doll's Eye Reflex aka Vestibule-ocular-reflex
learn normal and abnormal reflex , #video on a patient
the vestibulo-ocular reflex and it's demonstration on a normal patient.
*see Absent doll's eye in a normal and diseased patient.
0:30min--vestibulo-ocular pathway ( know it to understand it’s application)
2:00min--Oculocephalic reflex procedure : learn the basics
3:00 min-- Demonstration on a normal patient : Do you know what is a normal reflex?
    3:40min--Absent doll's eye reflex : finally,the abnormal part.
Vestibulo-ocular Reflex serves to move the eyes at an equal velocity but in the direction opposite to head movement.
This keeps the eyes still in space and maintains visual fixation while the head is in motion.
There are several ways to examine the VOR, including the doll’s eye test, head thrust test, dynamic visual acuity, and calorics.
The Medial longitudinal fasciculus (MLF) is the crucial fibre pathway that is important for this reflex to work properly.
It can be damaged in brainstem lesions( stroke).
It is used for neurological examination of cranial nerves 3, 6, and 8, the reflex arc including brainstem nuclei, and overall gross brainstem function.
Turning the head in one direction causes the eyes to turn in the opposite direction.
This response indicates that the pathways connecting the vestibular nuclei in the medulla to the extraocular nuclei in the pons and midbrain functioning and that the brainstem is intact.

The second case of absent oc reflex eg. I felt like the patient eyes were moving in opposite direction to the head movt.. Can some one please help with an explanation

Ist case is a typical case where there is no eye movement at all .
In second case, you can observe that both the eyeballs are slightly abducted in primary position of gaze. There is subtle movement of right eye only as compared to normal subject which disappeared on further follow up. ( Not shown in this video). Other brainstem reflexes were also absent in both the pateints.
Purpose of showing second case was that sometimes you don't get clear cut findings.

But the second (assumed dead) patient's doll eye reflex is positive. Observe the Right eye.

    • @NEURONBUNDLE
      @NEURONBUNDLE  4 місяці тому +1

Right eye appears to move a bit. But it isn’t significant. So, reflex is negative . ( for positive reflex, both eyes should move in opposite direction to head movement).

Great and simplified explanation. Just a small correction ( no offense), instead of using Normal PATIENT, it's better to say Normal/ healthy PERSON. Thank you.
