Re the intermittent proptosis (correct me if I'm wrong! I'm only a junior medical student): Intermittent proptosis occurs in the average human eye when bending forwards due to the action of gravity. If it occurs beyond the normal limits, it can be due to venous congestion (e.g. orbital varix). This is because there is stasis in the orbital venous system due to e.g. bending down (gravity) or hyperextension of the neck (obstructing the drainage). When those pressures are relieved, the proptosis goes back. Second reason why proptosis could happen might be e.g. orbital tissue laxity and this would only occur when bending down and not be related to the venous congestion.
