The only time I’ve ever seen a ref stop a grappling match is if someone is unconscious or bleeding. At some low level tournaments the ref sometimes stops it if somethings obviously broken. For the small kids, pretty much as soon as the submission is locked in the ref will stop it.
In kids jiu-jitsu tournaments, the referee stops it, and it actually expected of him to make a judgement call on whether the submission will succeed, a little while before it is even on, since the kids won't always tap, and thereby protecting them. For adults, the referee does not interfere until there is an actual tap, either by hands, feet, or verbal. If a guy screams in agony, it is also assumed that he has tapped, which in this case, you can see the guy is trying hard to keep his composure. My guess is, severe grimmace from pain would constitute the same. As an adult you're responsible for your own safety in a tournament - the referee only stops it, as some other comments aluded to, when you're unconsious, or there are bodily fluids involved, blood, vomit, urine, etc. because at that point you've lost control of your bodily functions. If you look at the gordon fight, his foot popped, but he also kept a straight face, for the submission to not count.
I think there needs to be a new rule if the ref thinks something is obviously broken and the uke is just not tapping from willpower(sometime stupidity) or adrenaline, pause the fight and call in a medic to examine
The rule could state that the ref have authority to call for a doctor when he is sure beyond a reasonable doubt that something is broken. Worst case scenario they reset them back to how they were. For the safety of the fighters I would be all for this rule being added.
