Thanks Susanna! What about nitrates? I'm trying to revise CHF atm and one source says that nitrates (and maybe ACEis) are definitely the best... then once you've decreased your afterload with nitrates or ACEi you'll get better perfusion to your kidneys and ONLY THEN will the diuretics really work. I think I'll need to find some clear guidelines, because it seems like everyone has a different way of managing it!

Currently in congestive hear failure take all the meds you described plus entresto can you tell me what entresto does for heart failure?

1:38 were valve and coronary artery even mentioned in the earlier videos in the progression of heart failure?
Also I never heard you mention that heart rhythm is a problem or cause of heart failure, regarding pacemaker. These sound like they might increase pumping of the heart, separately from what is actually wrong with the heart in heart failure.
2:22 could the load on the heart be lessened: again searching for solutions away from the problem. If blood pressure is too high, in most cases that’s arteriosclerosis. I don’t see what the latter has to do with the condition of heart failure. Rather the heart seems to be quite capable to force high blood pressure.
That’s literally saying let’s reduce the pumping function of the heart further, the exact opposite of what you want in heart failure. Especially since none of these interventions have been shown to stop progression.
(Just discovered there are stretch receptors in the atrium which are used to balance fluid levels. So staying a away from atrial ablation surgery is probably a good idea).
On top of all this, ace inhibitors to decrease blood pressure actually cause edema (which in this case obviously has nothing to do with fluid overload, since it’s caused by lax vasculature) another ‘sign’ that doctors will want to treat.
Fluid reduction: the only provable reason for potential of fluid overload seems to be showing severe kidneys disfunction. Edema often means very different things than fluid overload. Only severe heart failure, resulting in kidney failure, causes edema due to failure to regulate fluid. Mild heart failure usually doesn’t cause edema.
I seriously think they are just calling ckd, which will lead to episodes of fluid overload, congestive heart failure. They don’t even have a word for it wrt the kidney, maybe ‘we have a patient with ckd fluid overload’??
Around 7:10 you say Aldosterone will decrease "Fluid Overload" but doesn't Aldosterone promote the excretion of Na and H2O from the body? Therefore a reduction of Aldosterone would create MORE fluid in the bloodstream, right?
Beta blockers also block the release of renin as an added effect of this medication in reducing BP

I'm confused as to why both vasodilators and vasopressors are potential treatments for heart failure. Seems counterproductive.
I have watched all your videos on a 'heart failure 'and water pills works to reduce water retention from the body the doctors are prescribing but i was wondering does this means that the heart failure patient has to restrict water intake and if a patient take herbal " Dandelion" will not serve the same purpose .
