If you stick with it, you also get the administrative responsibilities of an Executive and the overtime pay of a Janitor.

      @Donna Henrikson School janitor here; the pay varies by district. For my district, it's $11.50 hourly so about $17.25 an hour overtime

    This is so real. My family doc has called me on a Sunday afternoon. He has driven (unasked) to my house to bring me a wax soak he was loaning me for an injured hand. He has been heartbroken when a patient died, and he hugged me when I had a terrible thing happen in my life. The family medicine doctor is part of his or her patients' families. He or she can see them from birth to adulthood. And these docs work their asses off for their patients.
    Fortunately, my doc has recently acquired a Jonathan, and she's fantastic!

      Mine never does anything to help me. He orders labs and other tests and then might send me to a specialist. He looks depressed and like he would rather be any place else than the examination room. I find it unhelpful as well as depressing. I get no help but I'm supposed to feel sorry for him?

      @@zaggardbrondie1147 This depends on your country. In mine (Canada) GP/general practitioner means the same thing as family doctor. Other synonyms include family physician, primary care physician, and generalist (though generalist can also refer to other specialties of medicine that also practice medicine from multiple domains). Family medicine is also recognized as being a speciality itself - to specialize in the general care of everything.

    Family physicians are the absolute best. In my country (Netherlands) they are absolutely vital. They are the gatekeepers of the healthcare system, easily accessible, treat the whole family cradle to grave and are the managers of your overall care. And the numbers don't lie, in the US the ratio of emergency physicians - family physicians is 1:3, in my country it is 1:21. They keep costs and workload down for the entire healthcare system. True heroes!

      This for some reason seems to elude the minds of most people in the healthcare debate. They just assume that a healthcare system fostered in a racially and culturally homogenous population, with minimal social and fiscal detriments of health, will just magically translate perfectly into a massive melting pot of differing cultures, beliefs and races, all of whom have a widely varying degrees of need and definitions of what it means to be healthy.
      And if you ever mention the above, you get labeled a racist. Which just goes to show how vested these people are in intelligently arguing their stances and the sheer magnitude of their grip on reality. 🙄

      @@cbl6520 As someone from Australia, which is definitely a melting pot of cultures and beliefs, stop with the bullshit, your healthcare system is decently below average for any comparable country for healthcare quality and access as of 2016 (results published 2018), with the pandemic that number is sure to have plummeted even further given the financial stress on people. maybe take a hint from the rest of the world and fight to overhaul half your systems which just exploit your population's continued existence or spend even a fraction of your ludicrously over-inflated defence budget on healthcare, education and social benefits and you might catch up to the rest of the world in quality of life instead of having to scrape for basic fucking health care, literally everyone I know would happily pay higher taxes if it meant they had to pay less when sick, even with the cost of living crisis.

      That's all the more reason WHY USA shouldn't be so far behind. It isn't the lower population that makes European healthcare better, it's the amount of taxes the population pays and to which degree those taxes are distributed into healthcare. Those things are much easier to scale *up* than scale down. Slap a 40% tax on Bezos and his cash alone could probably fund the entire health care system.
      (and let's be clear, the "homogenous European country" fantasy hasn't been real for many decades now)
      EDIT: and just to be clear: I am not chewing you out because I think you're a racist or any such bs, I'm chewing you out because the way Americans defend their shitty health care, shitty public schools, shitty public transport etc with "well it can't be helped, we MUST be shitty because we're BIG" is the same kind of bad argument you'll hear from an abused spouse. You've all been beaten down so hard by your government that you see it as natural to be slapped around, and you couldn't possibly have a healthy relationship where you're not beaten on a daily basis. It's very tragic and frustrating to watch.
      Other countries does not magically have better health care because we're smaller, it's something we had to actively work towards.

    My GP died recently at 74. He was much loved. One of the most caring human beings on the planet. He would work all day till 5pm then head over to another office and work till 9pm. His patients had his cell phone number. RIP Dr. H. You are greatly missed both as a person and as a doctor. It was an honor to know you.

      It's got a life of it's own.

    I'm a General Practitioner (GP) in Australia, and I believe I would cover the work of a family medicine doctor. It is absolutely INSANE how the profession is structured in the USA to make these doctors feel so overburdened and under-appreciated. I left a surgical pathway for GP and have never had such a wonderful work/life balance, sense of meaning in my work and appreciation for my job and patients (most of the time). More than that, I'm remunerated very well for it, compared to the median salary of a family medicine doctor in the US. Don't get a new job, organise and fix the broken system, because it can work beautifully for you and your patients.

    As someone who works in an ER, the cafeteria coupons hit a special kind of nerve.
    Recently, we’re given a coupon for 30% in our gift shop. Thing is, it excludes food, drinks, flowers, magazines, paperback and puzzle books, and greeting cards. Plus the gift shop is usually closed by the time my mid afternoon shift starts. 🤦‍♀️ Really makes you feel appreciated.

    I did (and taught) Family Medicine for 18 years after residency. It finally wore me out; I was accepted into a fellowship and became a subspecialist. Life is MUCH better now.
    If I had had a Jonathon, I might have been able to continue in Family Medicine. But Family Medicine is really a young doctor’s job.

    As a current Jonathan for a Family Med Physician, I can't imagine being a doc creating perfect notes that actually get reimbursed for the work and time you spent with the patient on top of all of the medical decision making. Not to mention the never ending in-baskets from patients and insurance companies calling the office throughout the day eating up the already 0 minutes between patient visits. Docs and Jonathans of this world, you are in my prayers.

    I always feel extremely sorry for Family Medicine.
    A few years ago I had the chance to become a kindergarten teacher and already had a year experience, because I did a voluntary year in a kindergarten.
    In this one year I've witnessed so many things...for example the way parents treat their children. A three year old girl sat in front of me and told me how much her body hurts every time she had ballet lessons. A THREE year old girl. The parents knew about that. It felt like as if at least 75% of the parents were awful. Most parents there had very fancy jobs. Lawyers, scientists, doctors, CEOs etc..
    Watching parents treat their own children like a piece of po*p and not being able to do anything about it is one of the most awful feelings to exist. That's why I decided to not work with children, even though it's one of the things I'm really good at.
    And that's why I have huge respect for every single person that works with children or people in general.

    PCPs are as close to social workers as medicine gets outside the mental health department.
    So determined to help everyone they can't say no and end up in worse shape than their patients/clients by the time they retire it's truly worrisome.

    From my decade in child welfare law this hits home, even though this video is medicine. The file up and down prep before and after each day’s court list was never ending and exhausting, usually about 40 kids a day, and I called them my kids-an overall caseload of around 300 in a courtroom of medically needy and special needs kids from 0-21, me trying to learn all about their medical conditions to make solid recommendations to the court because these were the kids whose families didn’t stick around to stay involved in their lives and make those calls. And our public interest salaries were cut as well when budgets got tough.

    My GP just retired last year. I still miss him because he was one of those good older docs who treated me, a Medicare patient, the same as the rest. If you don't know the difference, be glad you don't have Medicare. My doc had been suffering from burnout especially since the pandemic hit and it was just time for him to retire and rest. He earned it. I dragged my heels on picking a new GP, but I like the PA-C I finally saw last week. I think she and I will get along well.

    My PCP works three mornings a week as a part timer. And I’m okay with that. She is the best doctor I’ve ever had and actually is mindfully present during her appointments.

    During Covid I worked at the office of my family medicine doctor to support her in these demanding times. I'm about to finish medical training to become a family medicine doctor too. I have high hopes to work together with her and continue my doctor's office when she eventually retires (hopefully not soon).
    I know it's going to be demanding, so wish me luck ;-)

    I love the transformation when Jonathan arrived. He does paperwork AND grooming!
    For real though, if paperwork is such a time sink, why aren't medical scribes available for all specialties? You don't need a medical degree for it, do you? And having a second set of eyes and ears would cut down on mistakes. Prevent overwork, increase accuracy and detail in records, open up a lot of new jobs, get people interested in the medical field and potentially increase the number of future doctors and nurses in the process?

    My parent worked for a Family Medicine physician for about 15-20 years.
    We could always tell when he was around because he had a heavy limp.
    Doc blew his knee out and couldn't/wouldn't take the time to let it recover, then he couldn't/wouldn't attend his own physical therapy appointments. Then, when it got worse he couldn't/wouldn't take time off to get the corrective surgery.
    The man has threatened to retire for the last 20 years, all while definitely not retiring and angrily dragging around that painful cluster of nerves that he calls a leg.
    I swear, Family Medicine Doctors work like mules and are twice as obstinate.
    When people say, "Doctors are the worst patients." they mean Family Medicine doctors.

      @@RG-or3hu Family Medicine, where the Doctors all have blown out knees/hips and the nurses all have carpal tunnel/tendonitis and not one of them follows the advice they exasperatedly give to their patients.

    I show all my scribes (and we have several- twelve Ortho Bros at my clinic) LOVE the Jonathon skits.
    When the surgeons are long gone from clinic, they stay buried to their chins in charts... never smiling, eating, or interacting with anyone...
    I always get gut laughs when I slide up beside them and turn on your channel.
    Ortho is heavy work in an area that prizes sports above all else (from peewee to collegiate)

    I was going to send this to my Doctor. But then at the end saying family medicine needs a new job... I will cry if my doctor ever quits. He can never see this.
    Also we all know Jonathan already broke into Family Medicine's house and finished his charting before he even showed up. That's really what the nod was about.

    Family Medicine is like that piece of lose metal you jam into a project as a "place holder" but it becomes incredibly imperative to the structural integrity of the place yet at the same time it's increasingly becoming more and more likely to snap from the stress of too much work and not enough planning to accommodate it, but it can't leave or everything falls apart

    When my primary care doctor of decades retired I cried my eyes out. He helped me through many crises. Please know how much we appreciate you! Now I'm really worried that Bill will become my new doctor.

    As an RN in internal med I continually see our MDs get worked to the bone. Primary care is sadly going down the tubes. MD burnout is real!

    I’m not a family medicine doc but this brought a tear to my eye. Now excuse while I admit 10-12 patients, consult every specialist, and still make it home in time to see my family, relax in my hot tub, and watch a little tv before turning in just to do it all over again for 7 days straight awaiting the 7 days off I will have after. Oh yes, the life of a hospitalist. It’s hard work but someone has to do it 😂.

    Yes! And thank you for this! This was a realistic scenario from my residency days and I only graduated 3 years ago. My program was unique in that our Family Med patients were managed by the Fam Med inpatient/call team from the time they hit the hospital doors, including when they arrived in the ED. The FM inpatient team or FM resident on call was paged to the ED to manage the patient there. And while you were busy if you got paged that a FM OB patient was admitted to L&D or even came into OB triage you had to go and manage that. Similar to our FM peds patients, where we were primary on peds ward and if paged, we had to go admit them. Which then meant we also did discharges. And then depending on the day of the week and rotation, you may still have to do clinic on the same day on which you were in hospital and figure out how to get charts done before having to be back in hospital the next morning at 6 AM.
    So thank you for this. I hated residency because of how ridicously hard it was but looking back on it, I realized that it better prepared you for what you wanted to do later on (options). So for those who only see their FM docs in clinic doing referrals or refills and think they don't do much more other than complain about charting you don't begin to know half of the story....
