I'm not familiar with the 'game' side of it, but I believe you could treat it as one if you import MIDI files into it and connect it to your piano to play while watching the notes fall. So people may use it to learn how to play songs. Alternatively you can use it to make your own videos like this, which is what a lot of UA-camrs do. Even if you just search "piano synthesia", you'll see a ton of channels using this program.
