    Does the idiom apply to private belongings such as headphones? I've only seen it used to refer to elevators, pay phones or vending machines

      Any idiom has its own spectrum of use. And you are right, basically this idiom can be heard in relation to the things that you listed. However, this can be applied to any thing that has stopped working.
      Here are some other examples we could find:
      "Approximately four sensors is out of order, and four send the wrong information."
      "The men's room is out of order."
      "The alarm system was out of order for the first time in two years."
      "She says the baseballs are out of order."
      "Lock signals are out of order, follow instructions."
