I like it because the only thing Iggy and Tom really have in common is the fact that they are moderately successful musicians. They don't know each other, they don't really have anything to say to each other, and they don't get along particularly well in this scene. It's perfect, and broken, and real.

The whole scene works on so many levels, anyway (Tom being a doctor and only part-time musician, the quit-smoking motive, that they're both "real life" figures that mix their real life identities into this fictional little story, etc.). The whole film goes through several different people meeting up and having conversations like this. Tom Waits and Iggy were just a clip from it. And yes, the film is good.
There's places in Germany (or at least Berlin) where you can smoke with your beer (or soda or coffee or whatever). Who'd have thought USA would be more restrictive than Germany? (Also, I think there's plenty of places in America (the continent, North and South) where it's okay to smoke inside :P )
I was on holiday in Mississippi and in Morgan Freeman's bar/restaurant, 'Ground Zero' smoking was aloud. Also in Atlanta there was a bar you could smoke in. I know you're talking about Cafes but still it's better than nothing :P

To me, Iggy has some hilarious facial expressions and mannerisms (such as the disgusted face he makes while checking out the Diner before Tom arrives), and Tom's dialogue (like the Medical Morning Story) is damn hilarious. Both combined make for the best scene in this movie to me, and one of the funniest ones ever in a different, off-beat kinda way.
Except for when Billy Gibbons met Prince at a bar in New York. Those two talked for hours about guitar. Billy said in an interview that he was impressed by Princes guitar skills 🤷♀️

If you want to see a similar scene such as this only closer to reality -- check the video document on UA-cam that was a filming of a dinner at a hotel that writer & Beat Generation icon William Burroughs had with artist Andy Warhol. You would think these two men would have much to say and share. But while they did speak, it was awkward and didn't tell you much. Burroughs was conversational but aloof and stiff. Warhol never took his headphones off and he was impressionable. A "fan" of Burroughs and a little star-struck especially when Burroughs autographs a book and then was asked to draw something in the book. Their main conversation was about chicken fried steak. Not very intellectual or artsy. Just as peculiar as this staged scene with Iggy & Tom -- which is a classic now. Many celebrities do not mix. The Beatles did not get off to a good start with Elvis Presley when they met in Bel Air in 1965 either. You would have thought they would have been enthusiastic about meeting each other and comparing notes -- but witnesses said it was quite a drag. Same happened when The Beach Boys met Elvis. Nothing. Apparently, because of all the fanfare Elvis was exposed to he never took rock music seriously after his initial successes. So, it's assumed Elvis had nothing in common with these musicians. I wonder if Tom & Iggy ever recorded anything together? Wrote together? The way David Bowie did with Iggy. No matter...not everyone who you'd think could get along famously...ever do.
