@ash evildead apparently Howard has always been a sweet and nice guy off the air. Even then. So Richard probably knew that he wasn't for real when acting so mean on the radio.
In his book, he said he dangled a canned ham and a bag of chips on fishing lines over two former fat people's heads. The funniest part was Howard writing, "Richard started crying, 'Howard, how could you?' Anyway, I guess his plan worked, since they didn't eat the food."

This Richard Simmons visit was downright depressing. Simmons comes in screaming on a histrionic tear, passive-agressively talking and singing over everybody, ignoring Howard's questions, and hyper-focusing on topics (and creepy dolls) that nobody cares about. His campy schtick wasn't always easy to take, and you can hear how frustrated Howard is trying to get a basic interview out of this manic man-child.
That's just Richard's thing and even when he came into to discuss serious topics (like trying to push a measure to combat childhood obesity) Howard and the crew just goofed on him and called him gay anyway, so might as well camp it up and have fun with it. This was back in the heyday of the show when it was all about outrageousness and not the present day show where Howard only cares about kissing celeb ass and pretending to be James Lipton. Besides, Richard was nowhere near as difficult as some of the other guests they had like the Jesus Twins or Elliot Offen.
The thing that annoyed me about Richard is his phoniness. Does he really think that WE think he's straight? He'd be much more interesting if he was honest and real. But the excessive energy, the phony tears, campiness, etc...It's all a cover for who he really is when he is by himself.

In one interview he was talking about being locked in a closet (literally), and the interviewer asks "When did you come out of the closet?" and he said "Well I've been out of the closet for a while now...", plus all his jokes on Letterman "Your tight end is here!" and Whose Line where he really gayed it up. He's just extremely catholic, non-political, and not open sexually.
