One correction, the card is also available with Visa.
It started off with Mastercard and Amex variants only, but now is, starting giving Visa option too.

As per the bank, to get this card LTF, one needs to maintain a total relationship of ₹ 2 Cr with the bank.
The value seems a bit high but generally the luxury cards give a lot of rewards so the bank needs to ensure that they are getting enough money from banking. You can negotiate the 2 crore amount to a lower one to get LTF
Agreed. However, upon requesting and maintaining a good nrv, salary account etc we can reduce the 2 crore requirement.

Very poor in terms of rewards. There is no renewal benefit against the fee paid, either. Not worth ₹ 12,000

it depends on your credit profile, cibil score etc. You can request the bank for the limit you want and given your profile they should increase it
