    Data management is excellent in South Africa. The country is also extremely open regarding all its Covid related data. The doctor who discovered Omicron and several others are saying that, so far, this is presenting in patients as a mild form of the disease. Common side effects seem to be a cough, ‘scratchy sore throat and aching bones.

    Good on SA for alerting the world to a new variant, shame the knee jerk reaction to ban travel, when it clearly was out in the world already, if you punish the messenger, they will just keep quiet.

      Not sure I agree with your travel comment. I think it is a wide choice Christmas is the time where large family and work gatherings take place and people travel across the world l, socialising indoors more. So I think travel restrictions are the only way.

    The only question worth answering is how dangerous is it? If it is about as dangerous as a cold, then why worry? Why worry about jabs for this? We won't know for a while how dangerous it is, so surely, stop guessing?

      @@autistonaut do you know what was in any of the medication you've ever taken. There comes a point where you put your trust in those in the know. And depending on your circumstances take the risk assessment. If you feel you'd be fine, so be it. My only gripe is with false information being thrown about, and some people have died due to this.

    Let's pretend we are the viruses and not the hosts for a while. The mutations occur randomly so variants pop up all the time. This is our very own nature. Some variants are defective in the ability to infect a cell, others to make copies of their genes and they disappear. Other variants gain new abilities and might be more efficient in infecting or making copies of their genes. These variants will compete with each other and the ones that best fit the purpose of infection/reproduction will spread through people's respiratory (maybe other) fluids. As long as people keep exchanging their body fluids, we (the viruses) will keep evolving and causing many casualties (humans), but we will make sure they spread us before they die so we don't face extinction. Which ones will be the casualties depend on their own level of responsibility/stupidity. Eventually some of us will have mutations that allow us to infect, reproduce, and stay hidden in some humans or other animals without causing too much fuss. Now and then we will cause some fuss though, it's in our ever changing nature.
    By the way, we (the viruses) hate social distancing, masks and clean hands. Vaccines? They're a pain in the arse but as long as some people can't or won't have them, we will survive.

    Hey Prof. Spector how about getting onto those in power to change vaccination protocol to aspirate the needle. This was common practice for intramuscular injections. This would ensure no blood vessel is being hit, the vaccine not directly entering the blood system, and thus not potentially causing thromboses.

      Thrombocytopenia events associated with adenovirus vector vaccines were known and investigated nearly 2 decades ago.
      Nothing to do with aspirating, although hitting a blood vessel would be the worst thing to do with it.

      This is most important for the mRNA vaccine. Aspirating would take almost no time and remove a possible vaccination problem.

      If aspiration had been the norm, as it used to be, then the sequelae would have been no thrombosis, reduction of vaccine hesitancy, wider use of the AZ vaccine and no restrictions on its use in under 40’s. In respect of the last point, younger people, particularly young men have higher upper arm muscle mass and hence more vascularisation and more chance of hitting a blood vessel.

    Last question, could government guidelines provide advice on letting fresh air in, as many businesses keep windows shut as opening them isn't in government guidelines? Surely fresh air reduces spread with mask-use indoors in confined spaces, crowds and unventilated spaces? Ie ventilate if you can?

    Is the AZ vaccine perhaps more effective on mutations that have changes on the spike protein? I don't know enough about the topic to know whether all mutations concern spike protein or perhaps other characteristics of viruses.

    If the Omicron variant was to quickly spread in concurrence with the Delta variant's slow decline then the use of PCR tests which do not detect the S-protein rejection may become problematic to the extent that the spread of the new variant may be increased. People will need to know for sure which variant they have because many will believe that if they haven't been told they have been infected with the Omicron variant then they won't be carrying it.
    Reports suggest that the current UK PCR testing system only has about half the capacity to potentially identify the new variant at the time of the test because only 50% of tests can detect the s-protein rejection which would point to the probable presence of the Omicron variant.
    If half or so of individuals are told they may have the variant the other half will assume otherwise and then we're left with infections being driven by human behaviour based on poor public health communication. This happened with the use of lateral flow tests. This time the consequences of unclear messaging could potentially be more problematic.

      "Variants Variants Variants" or as I prefer to call them political scariants to coerce more vaccines which will allegedly last all of a few months before the next convenient mutation.
      All you need to worry about (or not) as is the case over 99.5% of the time is whether you have Covid, it is roughly the same virus it has always been just as a mutation of Flu is still the Flu, it won't suddenly morph into an apocalyptic horror show of a virus like out of some scary movie.

    I'm wondering if those mutations beyond the common ones that have been seen frequently in other successful variants and known to allow more transmission and infectivity are just "noise" and non-impactful to the virus' operation - albeit acting to making it less recognisable by spike-focused anti-bodies. If these mutations were materially "useful" wouldn't we have seen them cropping up time and time again in other variants? Should any "adjusted" vaccines also target NME proteins too to make them less vulnerable to mutations of the S proteins? Maybe an old-fashioned "inactivated virus" vaccine might be a good idea at this time?

      The moderna I believe is the old fashioned style serum. However people are getting a pick-and-mix option, where they can no longer choose to have that one alone. There is a mixture being applied to people. What will that do? The only way to find out is wait, I suppose.

      @@MElaughs Moderna is an mRNA vaccine. Very similar to the Pfizer one. Gets the cells to build fragments of the SARS-CoV-2 "wildtype" spike (S) protein.

    Local guidance has said we all need to start wearing masks in shops again after tomorrow.
    My Dad asked if I wanted to go to the Pantomime in two weeks time and when I looked at the theatre’s website they didn’t have any covid restrictions in place so I declined whereas the theatre near me has every precaution in place.

    To follow up on your comment that monoclonal antibodies do not work. This is in contrast to reports from USA and I quote TWiV . Is it because we are not administering them early enough ?

    Can you control a pandemic with international air travel ? Previous pandemics Pieter out after a few years. However now today a variant is found and it has already seeded worldwide. In all cases it is spread by air travel. The present infrastructure at airports spreads the virus.

      Communication is instantaneous, aircraft arrive all the time, so the time between the message arriving and the virus arriving is pretty much the same with both systems, its in the moment.
      Sorry when I mean both systems I mean the old sea transport days and now.

    How did all those people manage to travel whilst being infected? Were they vaccinated and symptomless, or did they fail to do their pre-travel testing properly??

      Interesting to know if all people test negative boarding a plane. On arrival how many test positive ? They must have the data.

      There's a delay in infection and testing positive, as the virus has to replicate to levels high enough to be picked up by a test. So people can get infected a day or two before traveling, and not show a positive test until after they're in a new country. This is what happened to the Israeli doctor who got Omicron from the UK, then passed it to his colleague in Israel.

    Yes, it was first detected in South Africa, but a swab taken in October in Nigeria was also found to have the variant.

    Isn't the point of a vaccine to. be immune support to recognise and fight a new, unfamiliar virus or infection? Boosters seem to try to do the immune system's work for it.
    Surely studying immune responses to community exposure after first dose would be useful?

    From what I have heard thus far, this new variant appears to have mild symptoms. I wonder how we define mild or moderate? For example, about eight years ago, I lost my job and health insurance, so could not get a flu shot. I ended up getting whatever strain of flu was going around at the time and the symptoms were so horrible that I truly thought I was going to die. It took me almost a month to recover, as was the case with many others in my social circle. Although I did not end up in the hospital, I did see a doctor, had a positive flu test and was given Tamiflu. My personal misery and that of my friends and colleagues certainly did not make news headlines, but I wonder if our symptoms would have been classified as mild or moderate?

    Monoclonal antibodies not much good? Here in U.S. they're saying they really help. Is your country using the same as U.S.? Please advise.

      Casirivimab and imdevimab as a combination are licensed in the UK, Ronapreve (Regeneron) is licensed in the US, so they are different. I don’t know much about either but I think in the UK it’s only given to those currently who have immunodeficiency and contract covid, so it’s not authorised for use on general population. I think you’d possibly get it if you were admitted to hospital but it’s not something you’d get from your family doctor.

      Not sure, but in a few countries many politicians, leaders, didn't want labs to test all the covid19 rtPCR samples that were sent to them, these labs were forced to destroy the tests and give false results, to obviously, keep their reported cases much lower than what they actually were, and hopefully gain more votes and undeserved praise world-wide.

      To give an example, yesterday my son and his daughter both tested positive for Covid. They have no idea which variant,. If they report that their symptoms are mild, there's no way to tell if it's due to having "mild Omicron" or if it's Delta and it's mild due to their vaccination. Maybe their strain was reported, and data will be used. But, probably not!

    One of my friends took Covid. He was in touch with a friend 8 days previously. It took this time for it to actually incubate. Is this normal?

      My granddaughter was diagnosed yesterday. They asked for her contacts for the previous 10 days, so presumably it can incubate for that long.

      For the Delta variant the incubation period is 3 to 14 days with a mean of 5.2 days. There have been so outliers out to 21 days, but they are extremely rare.

    It’s possible this variant is going through my sons class. My son is now ill and has very similar symptoms to his classmate. Started with a headache and high temperature but not extremely high. And felt tired and dizzy. Day 2 - headache and temp was gone, sore throat started and very lethargic. Day 3 - sore throat, dizziness lifted and looking much better. His friend was very similar with 2-3 days of symptoms. No loss of taste or smell, no cold, no cough, no respiratory issues. We live in South Africa.

    My question is if Omicron turns out to be more mild and less deadly why does it matter if the vaccine works or not? I don't care if I catch a virus that will be gone in a few days and won't cause any harm. Shouldn't we be more patient and see the data before all of this panic?

      With all due respect, you and I don't matter very much. It's when it overwhelms health systems, or lowers life expectancy rates, action happens. They don't know if it Omicron is deadly or not yet. That being said, I'd really really hate to pass this on to anyone, especially more vulnerable people. Some people make a massive effort not to pass it on. It would be wonderful if the vaccines stopped transmission again and turned the tables on the virus.

    I’m not sure I want to “avoid catching omicron”. Once you’re vaccinated, if you’re young and healthy I think it’s probably approaching your duty to catch Covid and improve your own immune response. If it’s endemic, avoidance is ultimately unsustainable. There’s a finite amount of “catching it” needs to happen and I’m happy to do my bit.

      @@zillie8167 this is a philosophical point with which everyone is wrestling. But actually, its always been there, and is in every facet of our lives as individuals in a society. If I go skiing and break a leg, and need expensive orthopedics to fix it, that robs the nhs of resources it could use to pay for someone else’s cancer treatment, all of which is restricted to some degree on the basis of funding. All you have now is a very obvious, apparently direct version of the same problem, traditionally called “economics”. My point is actually more that both our scenarios are infact inevitable….we will all get it. You will probably, at some point, accidentally sit next to the old lady. If it’s inevitable, and the costs of delay are high, delay is irrational. Nobody has yet presented any science (and actually most of what is going on isn’t science and most of the talking heads are not really scientists) that explains an alternative. All we are doing is trying to stop the tide coming in. Pro tip - he drowned.

      The efficacy of the initial vaccine dose wanes over 6 months or so, by how much depends on the particular vaccine. Even with a 3rd or booster dose the increased percentage protection will be nullified by the Omicron variant. You will at least be protected against hospitalisation or death.

    The only thing that I can say for certain is that this virus is getting less deadly. A year ago the global case fatality rate was about 2.3 and today it's 1.99. The US Case Fatality was about 1.8 and today is 1.61. It's getting less deadly and will continue to be less deadly overtime.

      Assuming your figures are correct, that doesn't necessarily suggest that the virus itself is getting less deadly (although it might). Another factor is that the number of cases is much higher now in people who are not as clinically vulnerable. You can have just as many deaths but if you also have more survivors the case fatality rate goes down. You have to consider the number of deaths as well. Plus, of course, the non-death serious outcomes which obviously only affect survivors.

      @@Grim_Beard I think you might be right. Either way, I think it's time we acknowledge that we should stop panic about covid and get our heads in the game. That way we might actually find a way to treat this thing. Thus far we have been paralyzed with fear and that fear has gotten in the way of addressing the problem and saving lives. Although, I am aware that their is a school of thought that says we need to get people to panic because it might get people to take this thing seriously I don't subscribe to that point of view. In fact, I don't see any evidence that panic is getting people to take this virus more seriously. Quiet the opposite in fact. Most people when they see somebody panic will either confront that person directly and tell them to shut up or tune them out all together; which could explain why we are having a tough time getting people to take this thing seriously. Panic never works and it is the last thing we should be doing in a crisis.

      Speak to your doctor about being referred to speak to someone. Last year I did silver cloud which is an online course and this year I’m having over the phone counceling sessions which my employer is actually paying for (which is amazing and I appreciate very few employers would do that).
      Ask your doctor what’s available in your area and make sure you have your own support network of friends/family to meet regularly and go for a walk or FaceTime with you.

      Not really, incubation time appears to be longer for Omicron, but more people have natural resistance or have been vaccinated, so a direct comparison with the Delta variant is difficult.

      "More transmissable" means that more people will get infected if/when exposed, which means that at least a few more (than with Delta) will get severely ill. Other than that, there isn't any information yet to compare how much more (or less?) ill people might get from omicron vs Delta. Delta turned out to cause something like twice as many deaths and hospitalizations (per infection) as the original version, simply because it seemed to infect more cells in the human body, for a higher overall viral load in each infected person. In that sense, there is a risk that if omicron is more transmissible/infectious, it might also cause a higher viral load than delta (infecting more cells in each person before it can be fought off), and subsequently be more serious too - but there isn't any data yet. If it somehow doesn't trigger the inflammatory process the same way, it might be much milder and less serious than delta.
      Other than that, we don't have enough data yet about speed of infection either, or how rapidly people would get sick after exposure. Remember that a delayed onset of symptoms (longer incubation period) can actually be worse than a rapid onset, because it means that more people would be spreading the disease without realizing their infected status. This is why AIDS has killed 36 million people so far over the past four decades (while Covid19 has killed just over 5 million or so, officially).
