The defending unit needs to have loaded black powder weapons, bows, lances, pikes , or fixed bayonets. It also has to have 0-1 fatigue. The usual way to deny a defensive fire is to get two fatigue on a unit. The defending unit also needs line of sight to the charging unit. The easiest way to deny line of sight is to throw a stink pot, and charge through the smoke.

People do. There are published campaigns and it shouldn’t be hard to draw on history for campaign ideas.
