Good discussion as always.I am an MBBS graduate working in a tribal setup and I always face the dengue vs lepto dilemma during clinical practice and I rely on LFT most often to make a quick diagnosis and referral to a higher center. Coming to this discussion, the presenter made a comment about the patient being referred from another center with fever for 6 days + altered LFT and RFT if I am not mistaken but then the discussion did not cover the LFT part at all. My doubt is whether Fever + myalgia + icterus + oliguria leaning more towards lepto/scrub than dengue in the first place or does dengue will also have altered LFT initially?
Usually in dengue fever thrombocytopenia is seen about 5 days after fever onset with ot, pt derangement in lft. For lepto fever, myalgia thrombocytopenia with altered rft and lft (bilirubin levels ) usually.

Yes dengue can have left alterations
