  • In Dr Tim's recent webinar with Dr Mobin Master (link below), we spoke about the unfortunate tear trough complication after injectors place filler into the orbit instead of the pre zygomatic space.
The effects of this problem can be nasty, long lasting, and very hard to reverse, so prevention is key. So, we decided to dedicate a whole Aesthetics Mastery Show to explaining the vital anatomy behind this problem, and the injection technique you need to avoid injecting within the orbit.
Pay careful attention as Dr Tim uses 'Bruce' the anatomy skull, to give you a detailed tear trough anatomy tutorial.

Dr Tim Pearce may I ask why not inject away from the nose towards the outer corner?

What are the expected long-term complications from injecting hyaluronic acid based fillers in the orbit? I am sure my cosmetic surgeon did this to me about a year ago. I felt it! Back then I thought I imagined it as I thought it was probably impossile. I have no ill effects that I know of at this time.
I just happened to have stumbled upon your video.

@Dr Tim Pearce and team, thank you for another interesting video. No need for US to inject through septum in experienced hands, as similar techniques are used in Ophthalmology for routine procedures. Regards from an Oculoplastics colleague.

When using a cannula, wouldn't the orbicularis oculi retaining ligament offer some (even if little) palpable resistance? And wouldn't the same go for the septum? Would that soft palpation with the cannula guide us through safer placement of filler?

What would be the issues you mentioned would be a problem years later, if filler enters the orbit area?

Yes, I’d like to know that too. I just had some under eye filler with a cannula a few days ago, and a day and a half after having it done I developed an eye infection. I’m on day three of using antibiotic eye drops and the redness in my eye has improved somewhat, but it is still swollen and irritated. I hope no filler got in my orbital rim😱.

What if it's the orbit that is sunken in? I had filler under my eyes and it was placed along the tear trough and such but my eyes still look sunken in because I seem to have lost all the fat in the orbit. All that money spent and I still don't have the results I hoped for. Is there anything that can be safely placed in the orbit?

Dr, I had 2 units of Botox into my crows feet and three units along my lower eye from medial to lateral. I have experienced puffiness, eye bags and what seems to be a droopy lower lid that fell to my cheeks. It has been 5 months since injection and only the puffiness has improved. I still see worse bulging creases when I smile (worse than before) and lines that trace my orbital rim to my cheeks. What should I expect now? What do you think happened and do you think this is reversible? Thank you very much.

hi doctor, I am a 26 yo french guy who gets a huge under eye wrinkle right under my lower eyelid. I was wondering if botox could help? but this is really close to the under eyelid. thanks

Hi Eudes. Thanks for your question. Miranda here. Botox doesn't tend to be great for directly under the eye, because it works by relaxing muscles, for examples the muscle that creates crows' feet at the sides of the eye, whereas your under-eye line probably is not created by a muscle. Filler can work under the eye by filling in the 'trough' or indentation, but it doesn't always help lines. I recommend going to a local clinician near to you and having a consultation to see what can be done. All the best, Miranda
