If you ever have a spectaor enter a lobby theres a good chance hes calling out you location to someone on the other team so if you keep getting killed in spots where you know they can't see you, that might be it.

I play at default for all settings except my reticle size, which I believe I have around 25%.

100% agree, however, later I found out this individual got stuck with the Invincibility Glitch that was going around for a little bit.
