Not exactly visa application only requires the printed details on the first page of your passport Apart From m that any details you submitted to get your passport can be changed when applying for visa

1. If fully sponsored, chose US company and put your school's name there. 2 You can tick not apply with NIN or use your Nigerian NIN

Use a photographer who understands the dimensions. Visa fee is what you pay at GT bank, not sevis
