How to Use a TENS / EMS Unit for Pain Relief

  • TENS units & EMS units can be helpful when recovering from an injury or surgery. The two are very different, but often come together in one unit like the iStim EV-805 unit I use in this video.

    What's the difference between TENS and EMS?
    TENS (Transcutaneous Electrical Nerve Stimulation) is used to help reduce pain and increase circulation. Basically the vibration of the TENS follows the same pathway as the pain pathway to the brain and helps cancel it out. It is a great alternative to pain medication. A TENS uses four electrodes with cross currents to surround the area. The deep vibration/massage helps relax the muscles and allow for healing.
    EMS (Electrical Muscle Stimulation) helps contract the muscles to build back up strength after an injury. With EMS the muscles will actually contract, and you can use it as a tool while exercising to help retrain the muscle to work how it's suppose to. It uses two electrodes directly over the muscle you are trying to contract.
    The iStim EV-805 I use in this video is a 4 Channel TENS+EMS Combo Unit and comes with 24 Replacement Electrodes Pads. It has 5 TENS Modes + 3 EMS Modes + 24 Preset Programs. You can also manually set up the pulse rate and pulse width if you want to make it more specific for your needs.

    Doctor Jo, truly you can’t know how perfectly aligned and on time you are with/for me. For weeks, I’d mention something to my mom or a friend, and within 48hrs you’d upload a video regarding the exact thing I mentioned.     I just want to ask something, so I'm trying to do the splits and am following some guys tips. It's been quite okay but when I stretch my legs I feel something move near the knee. It doesn't hurt it's just weird. So is it because my lack of flexibility or is it normal?

    Hi, I play lots of football and not too recently, I injured my ankle. What exercises would help me fix this pain. The pain is underneath my inner ankle. I find it difficult doing calf raises with my right leg. Thanks

    Would a tens machine help for left C3/4 dysaesthesia facet joint arthropathy in my skull due to degenerative dis disease please?
    Also any exercises to help relieve the constant pain I'm in and have been for 3 months now especially worse at night, please? Can't get a MRI scan and CT done for 2 months. Any pain relief would be helpful thanks

    Thanks for defining the difference between the 2 modes. I just got a new unit that is both TENS EMS because I need deep muscle pain relief and the plain TENS wasn’t cutting it. When I tried it first I was criss crossing like you’d taught me years ago and it felt good but I was frustrated it didn’t help better with the pain. I will be trying to target the muscle with each channel for EMS and see if that helps.
    I’m trying next to decipher what mode is best for mid back pain, shoulder pain, neck pain. My machine shows a diagram of various exercises deadlifts, incline crunches set so I’m using the Overhead press and then a rate that bounces between 05 and 77 Hz while the pulse remains constant at 170 Hz. but honestly have no idea what is best. Do you have a video that describes what these 2 Hz readings mean in the TENS mode and EMS mode⁉️
    You are the ONLY person I ever recommend when someone complains about pain and I explain how my PT encouraged me to buy a TENS years ago. Thank YOU 🙏🏻 for all your efforts to help us find relief at home for chronic pain‼️

      You're welcome, and thanks for recommending my channel to people! Most of the units that have preset modes are kind of stuck to exactly what you mentioned, and unfortunately, are not always accurate in their descriptions. So often, I recommend just using the one that feels the best or feels like it's working what you want it to work, and not worry about the pictures. There are some machine you can buy that do have manual settings and you can program them how you want (like the ones you see in clinics, just not as expensive). If that is something you are interested in, I do have a book about TENS/EMS units that go into specific detail about them. The book also has a section of exercises using the unit.     After working out the next day I'm not sore nothing is hurting but when I do pushups to warm up my arms hurt really bad

    Dr Jo… I have back pain after a Lumbar Canal Stenosis operation. How many times per day I can use this TENS for this severe back pain? Thank you….

      When was your surgery? If your doctor has cleared you to use one after surgery, they can be used 3-5 times through out the day.

      TENS doesn't fix the problem, it helps control the pain so you can do the stretches and exercises you need to do to get better.

    I'm 20 years old and my MRI Sais : Mild bulge of L4-5 & L5-S1 disc indenting the theca.
    My pain always comes when I do any activity that requires core engagement * nearly everything at the gym*
    I'm Egyptian and we don't have good welling to help doctors .
    Could you please help me to get rid of that , I hope to live a normal life.

    Can you describe some of the "24 preset programs"? I can't find anything on that. Programs for recovery? Massage? Etc. Thanks!

    I would first like to apologise for writing a irrelevant comment here that's not related to the issue you're targeting. But wanted some advice on the sciatic nerve pain issue. Past week I did the stretches that you adviced in the video and they were great! So thank you for that one. But when I was doing them I always felt numbness in the knee area. Like either it's the knee that's tingling or going numb, as I can't stretch the leg completely straight for much time. When I walk I feel numbness infront of the knee. So wanted to ask that can you advice something on this one, it'll be a great help. Sorry for such a long comment. And thank you for your help with so much of helpful content.

    My mom’s pain is behind her knee. Would the pads be applied on the back near the pain or would it be effective if applied on the front as you showed? Thank you in advance for your advice!

      It really depends on what specifically is causing her pain. Does she have a diagnosis from her doctor? In general, the pads go around the pain.

      It depends on where the injury is, and what specifically caused it. So it's best to check with your healthcare provider since everyone is different.

      No, not for the ones I am demonstrating. There are two different classes of EMS units, and some do require one, but not these.

      It sounds like your closed captions got turned on. You can easily turn them off by clicking the "CC" button at the bottom of the video.
