How Digoxin acts as Cardiotonic in Heart Failure?

- Digoxin increases force but decreases rate of contraction of heart. So it is specifically a cardiotonic with opposite action on the rate. It inhibits Na+/K+-ATPase pump thereby increases intracellular calcium levels which are responsible for increase in the force of contraction. The inhibitory effect on cardiac rate by Digoxin is due to its vagal activity.

Increase in which ion concentration causes digitalis toxicity sir?
Digitalis toxicity can be increased by raised levels of calcium within the serum. With the elevated levels of these ions they more enter into the cardiac membrane and thereby raise intracellular sodium levels resulting in ventricular arrhythmias. That's why lidocaine can be used to treat these digitalis induced arrhythmias by blocking sodium channels. Similarly, hypokalemia can also increase digoxin toxicity.

It's phase 4 brother . On phase 4 the memberane fully repolarized and became stable till the. Generating next action
Phase 4 is mainly observed in pacemaker cells where membrane potential is around -55 to -60 mv which slowly reaches to -40 mv by opening of calcium and sodium channels within phase 4.
