There was a timeline split where this left prototype stage and went into actual production which in turn, led to wrist-mount Bolter in 40k universe. Luckily, we are not in that timeline.
As much as I can see the Fallout parallels, I think a Warhammer 40k comparison is more fair to make. I think some variants of Space Marine Power Armor legit have wrist mounted bolters.
Some terminator and heavy devastator squads use a wrist mounted bolters to use a powerfist in the same hand. The Grey Knights use wrist mounted bolters as standard so they can use their two handed force glaives (and yes I cannot remember what it’s like to feel the touch of a woman)

Not really the same application. Much more practical though, and it would slot into the established world of fallout perfectly. Though a more practical application would angle the barrel slightly "down" from the wrist. This stands to reduce the potential recoil felt on the connective tissue of the elbow, shifting the strain to the muscles of the arm, and should make stable aiming easier. The reduced recoil discomfort might allow for a reduced harness, no longer taking up the whole arm, or perhaps eliminating the need for such an obtrusive handle resting in the palm, making other tasks easier. It could also allow for a springloaded "stock", due to the angle the arm would be held at, bringing the firing line back to where it would be on a more conventional weapon. The stock could be completely out of the way until a more precise shot was needed, and the right modern materials would barely add to the weight at all. If it was kept stockless, modern optics would greatly benefit the design as well, as with many SMGs and automatic pistols, which this would be loosely analogous to. You could also expand the weapon platform, since so much structure is already present, with things like tazers on the hand plate, a mounted switchblade, or even a small shield.
Not sure a ballistic fist actually fires anything? I think the idea behind it is it ignites gunpowder on contact causing an explosive small flash of force burn. You don't actually ever load the thing with anything. It just works until it wears out so it definitely doesn't use ammo.
