no canon death. galactic atlas got the character right in the next profile wrong, stating luminara unduli as a council member. galactic atlas is unreliable. then it is rendered obsolete when the following year had shaak ti and coleman kcaj's fates as "presumed killed" in the visual encyclopedia- the one specifically tying into all films which are canon and not just badly done art with weird headcanon. coleman kcaj is a confirmed survivor who made it 5 or 10 years into the purge before being hunted by the inquisitors. Yoda's vision also displaying everything wrong from order 66 with eeth koth instead of agen kolar being killed by sidious, clones in all white armour shooting red bolts to a group of jedi who are charging like they did in aotc at the arena battle, and saesee tiin with a blue lightsaber.. none of this happens, they were all in that same vision.. so why is shaak's appearance in there suddenly true and accurate then? its. not. consistent. and that's why we need to be better when we do reference sources, and if they are scant or contradictory, we must act honestly as sw lore enthusiasts and reserve judgement until something changes in a meaningful way.

I can add one more killing of her - as the purge reaches Camino, the order was called succsessfully executed with only one Jedi in place (Shaak Ti) before the place got bombarded and sunk. In one Clone Wars scene, her dead body (but with a covering blanket) was delivered to a gunship by clone troopers. So maybe she's indeed pretty tough, or made some nice side deals in her long time at the cloning facility =P

6:26 - Before the 2014 reboot the EU was canonical, although subject to revisions, retcons when compared to material considered more canonical, hence why Lucasfilm had a canon tier system. The EU was canonical but not as canonical as the films and TCW. What Dave Filoni said about what GL said isn't entirely true because T-canon only consisted of the TCW film and TV show, not the previous SW TV shows, and one of the many things TCW contradicts is 2003 CW. GL's opinion has no real weight on this just because he wasn't personally a fan of one thing or two from the EU. Sure, he created the franchise and did the films but if he didn't want that material to be written, produced and released as final products then he shouldn't let other people play in his sandbox. It's a useless and annoying discussion that has been going on since 2014. Her death is not described on the ROTS novelisation, seems like you haven't read it at all. Star Wars Insider (you know, the official magazine) issue 87 confirms that both deleted death scenes from the film are indeed non-canonical. I know this because, you know, I actually took time to get a copy of said issue and read it. What you see in TCW is some sort of vision or premonition of the future or a possible future, it's not an outright confirmation that that would be the exact way that Shaak Ti would die, it's a reference to the deleted scene only. The specific episode in which Yoda has the vision of Shaak Ti's death is "Voices", TCW season 6, episode 11. Dave Filoni has supervising director credits, Danny Keller has director credits and Christian Taylor has writer credits. Shaak Ti's death in Discanon is completely separate from Legends or pre-reboot canon.
