Re: the Tenerife disaster... St. Thomas is 5000km from Tenerife! The aircraft were being diverted to the airport where the disaster happened because of a bomb actually going off at another airport on Tenerife. A big contributor to the disaster was not only fog, but also miscommunication between the tower and the KLM jet thinking it had clearance to take-off. Great video though, I plan to use it in some shift training in the next couple weeks.
One small correction, Author Atul Guwande (in "The Checklist Manifesto") noted that the idea of CRM and checklist began in the military Oct 30, 1935 with the crash of the Model 299 aircraft (Later to be called the "Flying Fortress).
