Once a year or 10,000Km or depend on each situation. Actually if you have a apability to do it by yourself you can do it anytime you want

I never found any problem after use shimano gease with ceramic ball bearing but if you not sure you can use specific ceranic grease according to that wheel recommended

I use allen key 5mm 2 pieces and bearing extractor to remove freehub body bearing out in the EP.40.
