Flesh can easily be cut with a knife (scalpels are just really sharp and small knifes) which would make using a chainsaw excessive, also the teeth would likely cause unnecessary damage compared to a straight smooth blade by catching around the intended cut Meanwhile for a bone you need to put in more work, with a regular handsaw it takes quite a bit longer and you want to be quick between opening and closing the patients body, so a rotating blade is great (today we tend to use oscillating saws which i personally find to look even more terrifying)
