Why Carbidopa is given along with Levodopa

  • Carbidopa is a peripheral dopa decarboxylase inhibitor which inhibits the peripheral metabolism of levodopa resulting in its bio-availability within the CNS. But carbidopa alone is ineffective as neither it can cross the blood brain barrier nor increases dopamine levels within the CNS. Unlike Levodopa, Carbidopa is structurally not an amino acid therefore it is not transported into CNS by amino acid transporters.

      This was great, thanks, been searching for "what are the different types of parkinson's?" for a while now, and I think this has helped.     Great medication for Parkinson’s

      Hi George. bear with me as the explanation might get long. The only reason I can think of in giving Levodopa/Carbidopa to a patient with MS is to control the spasticity that can occur due to the lack of inhibition from upper motor neurons. Normally, upper motor neurons prevent lower motor neurons (aka alpha motor neurons) from over activating. These lower motor neurons are the ones responsible for causing your muscles to contract and therefore let you move your arms and legs at will. MS is a condition where your body builds antibodies to the oligodendrocytes (the cells that build myelin, a fatty insulator that helps speed up electrical impulses and makes communication between neurons more efficient) in your brain and spinal cord. These antibodies damage the myelin in upper motor neurons and therefore these neurons can't effectively control the lower motor neuron, creating unwanted contractions and tremors, similar to what occurs in Parkinson's disease. My guess for giving this drug to your wife is to help reduce or prevent any tremors she may experience since this drug increases the amount of dopamine in the brain, which in turn activates upper motor neurons. The doctor is probably hoping that by over-activating these neurons, enough electrical impulses build up to where they overcome the damage the antibodies are causing in the transmission of signals in the brain and between the upper and lower motor neurons. This treatment for MS was not something that was thought to us in medical school, however, so I think your wife's doctor wants to try a non-textbook method to control symptoms since most treatments for MS involve injections. I recommend asking her doctor their reasoning for giving it though, to have a clear mind.
