Treating an Inflamed Ilioinguinal Nerve

  • Neurogenic inflammation of the ilioinguinal nerve can cause pain on the anterior aspect of the thigh. This particular patient had anterior thigh pain that started after a car accident where the seatbelt most likely damaged his ilioinguinal nerve. We performed a nerve hydrodissection to help reduce the chronic neurogenic inflammation and improve pain.

    How I got here…

    23 yrs old: Competed as a long sprinter, anchoring the 4x400m relay to a silver medal at the Canadian national championships, contributing to Western's first national title in men's track and field.
    23 yrs old: Started working as a personal trainer for GoodLife Fitness in Canada, quickly becoming one of the Top 10 Personal Trainers nationwide.
    23 yrs old: Herniated two discs in my lower back, ending my track and field career and sending me into a multi-year battle with chronic pain.
    24 yrs old: Enrolled at the Southwest College of Naturopathic Medicine (now Sonoran University of Health Sciences) after discovering my passion for naturopathic medicine.
    26 yrs old: Still battling chronic back pain, I finally became pain-free through a platelet-rich plasma injection.
    28 yrs old: Graduated and become a licensed naturopathic doctor.
    29 yrs old: Completed the first-ever residency in regenerative medicine and integrative pain management at the Neil Riordan Center for Regenerative Medicine.

    Does one need to lose weight if they are obese to get a Genitofemoral nerve block? Are these nerve blocks done through the abdomen?

    Dear Drew. I have a condition where i dont get help from our med department. You see i had reflux and thatwhy i needed to sleep head risen up. Im also side sleeper, so i slept some weeks in really bad position. My legs started to hurt and after a while my legs started to swell a little. I assumed it was bloodclot so i went to hospital. They did all bloodwork, xrays and heart monitoring and nothing. No reason for leg swelling so they assumed it was vein insuffiency. I didnt hear them and i googled lymphatic massage because i feared it was some kind of startingpoint of edema. I did it and next day i had 1-2mm swelling / bloating all over my body..back, hands all over marks from sheets. I went back to hospital and they said its nothing..maybe too much salt. 3 moths later and less last in my diet nothing chaged. I thinked that my legpain and nerve tinglings in legs is due piriformis/sciatia compression. I went to massage. Massager got my pains and tingling away, but not swelling/bloating. He found out that my ilioinguinal nerve and inguinal band was "stuck". I didnt notice how tender that area really was.
    So i ask: can this nerve compression/stuck/block/entrapment/irritation cause swelling or lymphatic symptoms. Is there something i can tell our hospitals that can lead them in right way or do i just need to treat this in manual theraphy like massage.
    ps, i cannot use nsaids because im on caridiac aspirin

      This patient had about 50% improvement in symptoms with the treatment

    If hernia mesh has entrapped the ilioinguinal nerve is it best to remove the mesh Or do you only recommend that if nerve blocks fail? And can an entrapped nerve cause swelling of the testicle?

      testicular swelling would not be due to an entrapped nerve. If the nerve is trapped by scar tissue then our hydrodissection approaches often work. If it's truly the mesh then that's outside our scope and we would refer to surgeon for consultation

    Can someone get ilioinguinal nerve damage from ALIF surgery? Had ALIF 3 weeks ago and can only walk about 100 yards before my lower left side abdominal starts burning and aching very badly. The incision I had was right below the belly button so not sure what to do other than wait for the visit with the surgeon in 3 weeks. Thanks,

      minor nerve damage can occur from ALIF yes, but it could also be indirect irritation due to fascial adhesions from the surgery

      @@DrewTimmermansND Thanks Doc.... is an ULTRASOUND the best way to see if there was damage or fascial adhesion? What would you suggest for diagnostic. I met with my Back Surgeon yesterday 6 weeks post op and he said if it doesn't improve in 6 more weeks to see the Vascular Surgeon that did that part of the surgery? It always seems worse later in the day and after standing or walking more than 1/2 mile. Thanks and subscribed and following with a thumbs up!

    Hello Doctor, I had open inguinal hernia surgery in September 2020, since then I am having burning sensation, tingling, numbness near the groin area below the incision. The doctor is saying my ilioinguinal nerve got damaged because of the surgery. Please advise what my options are. Appreciate your help.

      I'm terribly sorry to hear that. In our clinic we perform ultrasound guided injections of prp or microfragmented adipose tissue to help the nerve heal.

      I had the same surgery in 2005 which has left me with constant pain and I tried injections but nothing worked....currently pregnant 19 weeks and in he most pain and had to go A & E only to be given paracetamol...no pain killer has worked since my operation..I just been abandoned by health professionals what should I do?

    sir my groin pain starts from a testicle trauma then gradually spreads to thigh , toe, feet, hamstring, buttock in 2 months but no pain in testicle, is it illioguinal or femoral nerve problem can you say?

    If an entrapped ilioinguinal nerve is causing pain from the hernia repair site down to the thigh can the condition improve over time on its own as some surgeons might claim?

      It can improve on it's own, but if it hasn't improved after 6 months we would evaluate you for nerve hydrodissection with PRP.

    After a C section and 2 repair surgeries...I had mine frozen to "reset" the nerve signal. It left me with a bad scar line to my pelvis and a droop to that side. Before that the doctors were just treating me with direct lidocaine.
