So how does the Double KO system work in this game? The first one awards a victory icon to no one, yet Sagat (2P) receives one when it happens again. My guess is that no victories are awarded unless a Double KO occurs in the third round AND either 1P or 2P does not have a victory icon leading up to the third round. So Adon would've received a "D" icon had Sagat won the second round.

Correct, there’s a max no of consecutive rounds which can be had. I believe it’s 4 in total with a standard 2 round win system.
