    A person with serious mental illness is demanded by our system to manage treatment themselves, regardless of delusion,persistent auditory hallucinations,paranoia and confused thinking. My son suffers with schizophrenia in Colorado. Our system has failed and his family has no where to turn. America 2018.

    My sister is severely ill with paranoid schizophrenia and she has been diagnosed by 3 doctors because she discontinues her medication whenever she feels her hallucination is under control after a few months of treatment and she would go to a different psychiatrist when hallucination breaks out again because even she knows that she shouldn't have discontinued her medication but she won't be punished for it. Who is getting punished? Me and my family who has to endure her daily emotional and occasionally physical abuse (because she has cut off all other relationships now and have thousands of reasons why these people betrayed her or had ill intentions). My parents don't want to admit that she is violent because they can't take the truth, and they even encourage her to discontinue her medication because they think she is not ill. I am sad to say that my sister is dangerous to the community and irresponsible parents like mine are going under the radar of our medical industry and social work programs. RIP DJ Jaffe. Thank you for your life-long dedication.

    I guess I would agree with you on most points if your main premise about "evidence based TXs" wasn't horribly wrong.

    When it comes to mental illness many ignore it since many feel helpless and hopeless at the system while trying to navigate within the frustrating systems. There are other alternative options to help advocate and continue to be driven by sending letters to your assemblyman and women within congress at a local level, senators, house of representatives. Be a part of the change that takes a village in order to improve the quality of life for all. Be a part of the solution and find out how to become involved in advocacy groups towards the same cause and learn about the barriers from the opponents in order to bring it everyone's attention to developing a change in society.

    While there are very good points, his commentary on trauma was distressing. Yes we all experience trauma, and it is a relative experience, however there is an increasing body of research which suggests that the effects of trauma need to be considered as a major environmental challenge that places individual’s physical and psychological health equally at risk; this is essential factor to consider when developing treatments

      John Dittmann I agree whole-heartedly with this. I understand his point, about how serious mental illness will most likely happen even in the absence of trauma, and how all these efforts to consider all risk factors is taking away from treating those already seriously ill.
      However, he's making it either/or, and it's very much both. Trauma absolutely contributes to and worsens mental health conditions, as you said the research is showing. It causes changes in the brain beyond just genetic predisposition. It's also a huge predictor of substance abuse, which makes mental illness even worse.
      He would be wise to understand that those make huge contributions into how severe mental illness can become.
