This depends on how drowsy. If she is not maintaining her airway even, you'd need to consider whether she is a candidate for intubation and ventilation on ITU (in this case, she is not given her co morbidities and functional baseline). This then leaves you with three options- medical treatment only (no NIV), medical treatment with NIV or palliative care only. This is a consideration and discussion you'd need to have with the patient's family - not all patients may want/be suited for/tolerate NIV in which case you'd just trial medical treatment but others might do in which case you trial NIV. This would be on a case by case basis after involving their family and your consultant. Hope that makes sense
