I was under the impression that they caused: - Ipsilateral sensorineural hearing loss with tinnitus - Ipsilateral lower motor neuron facial nerve palsy with no hyperacusis - Ipsilateral facial sensory loss (trigeminal nerve) - Rarely, in advanced cases: palsy of CN VI, IX and X But not homonymous hemianopia. Quite useful but I’m a bit surprised not to see Dissection/PE in the list of differentials before we obtain full history. Hypotension in a young hypertensive is always worrying.

Ace inhibitors are rarely associated with siadh. An endocrinologist I work with and respect doesn't even stop ace inhibitors in a case of siadh. All I'm saying is to be careful of attributing siadh to an ace inhibitor as it is unlikely to be the culprit.

As others have said the differential given was a bit odd. I don't know what stroke or SOL would cause generalised muscle weakness. MG was the obvious main differential given that presentiation

Paused the video; I was expecting selective BBB or CBB In the medication( their over dose can maifest with both hypoglycemia and hypotension), but the wife did not mention any so i would probably double check with the wife if she recalls any other medications, Addison crises is a unifying dx also so i would ask the wife of some similar episodes . right ventricular Mi with this profound hypotension definetly is on my list. Ruptured aortic aneurysm with significant internal bleeding can not be off my list Septic shock or hypovolemic shock all remained in the dd for me but the acceptable HR was keeping them away from the top of my list and the wife not mentioning any apparent illness lately... Ecg, examination, cbc, electrolytes would narrow down the list from the Er
