- Tips and tricks for new nurses during med pass!
- Always try to start a hour before med pass
- Make sure cart is stocked: water, pudding, thicken liquids, pudding
- check to see who has appts, baths, visits with family members, activities times, smoke breaks
- Dialysis (early breakfast tray): no BP meds
- How the patient take meds: crushed in pudding/apple sauce, thicken liquids, one at time/all together
- PRN pain meds schedule
- Have report sheet or notebook to chart PRNs or resident complaints/needs
- Med refill sheet
- Get Refrigerated meds before hand
- Get to know who likes meds early/first or last
- Tube feeders: have water ready and extra syringes just in case, decloggers available, stethoscope to check placement

Walking rounds are important. If the nurse you're relieving didn't do something you can tell them to do it before you count. This has saved me from multiple problems including finding a deceased resident and a missing resident. Also if you aren't comfortable asking you can arrive alittle early to get your assignment and just do a 10 second assessment on the room/resident and ask the nurse questions. For example for any of my residents who have something invasive like TF: i make sure they are sitting up, sometimes the aide will forget to sit them up. I look at the machine and tubing. How is it running, is it leaking and make a note of the flow rate so when the nurse giving report says its given at 45ml/hr on continuous and you noted it was 65ml/hr, make them change it and same with IV. This also gives the opportunity for you to observe your trach residents. You can tell that nurse that he/ she needs to be suctioned or trach care wasnt provided. Make sure there is water in the humidifier and there is sterile water and trach supplies in the room. Check your foley people, sometimes it wasnt drained and if they have urine in the bag notice color. If they dont have urine make sure you come back later to see if they've voided. This will help you decide if you should flush the foley, get a bladder scan and call the doctor/np.
You sound like a great nurse who managed your time well, which is my main struggle!! I used to work in the hospital and now am an agency RN, going to LTC facilities, so having 25+ patients is brand new to me. I was also used to the pyxis for getting out meds, which is so much faster and easier than these dang med carts! The only good thing is not having to run back and forth to the med room! I have mostly been doing nightshift until I become more efficient, but even with far less meds on that shift and starting around 4:30am, I am STILL running late and not done by 7am when the next shift comes in!! I feel like I do most of the things you mentioned, but not sure why it takes me so long to do the meds! One thing I do know of is the fact that I go to different facilities that all have their own way of doing things and agency nurses don't get orientation so I often don't know where things are and waste so much time searching for supplies and things. If you have any specific tips on how you were able to get your med pass done so quickly, please share if you wouldn't mind....would be SO grateful!!! I hope you're loving your hospital job!

Honestly it went horrible the first place I was at. I honestly thought I was gonna lose my licence and spent 15 hours for a 12 hour shift with all the documentation. However, this was simply because that place was super corrupt and had a 180k fine that quarter. They're one of the worst places in the city. Now I continue to work agency and it's not a big deal now. I wish there was a better way to research patient load and ratios for nurses and the long term care places but the two places I work most now aren't bad. And I make amazing money. Typically I work nights now just because the work load is basically cut in half on 11-7.

Yes I was on days for 2 weeks and then nights for 1 week. So really 3 weeks I guess😂😂 did you have prior experience?
