QotD: For me its not an item but pills in Issac. I am not the type of person who gets too down on a few bad pills. And being able to gulp trinkets and boost my tear stats is well worth the risk for me. Though I understand its not for everyone because one bad roll can look to ruin a run but hey a good roll can make it exponentially better so why not. I dont get as hung up on the bad rolls as other people might. As for other games idk but thats cause I dont really like playing to the exact meta and creating my own (mainly cause I like running the numbers and etc, and find just looking up guides online to be kind of meh. Like PoE I never made the "best" build but made the ones which were fun). Though I generally think most items in a game are meant to be good somehow, and I just need to find the right conditions for it to sing, which is mostly true. Idk the only thing I can really think of in GTA Online I would play with cars I found interesting or fun rather than just powerful cars. Like you could breeze through missions in a bulletproof Kumara or Duke, but sometimes just riding out in a hearse is more fun or doing the entire mission in a buggy and etc.

Shard of Glass for Isaac, frequently seen as a detrimental item... but has incredible rigging potential if you lean into it (also had a 7 hour mega rig that resulted in me unlocking Lost AND Tainted Lost in the same run due to D20ing the heart drops as Isaac into missing poster)
As for other games... Personal Shield Generator in ROR2. While it effectively reduces the percentage of HP you can heal, I jokingly did a run where I stacked it into oblivion and became nigh unkillable as I was decent at staying out of danger to regenerate the shield health.
Good early but I still hate it lol
