    When i first started trt on test enanthate, i had alot of estrogen conversion to the point where i felt just as crap as when my testosterone was low, my doctor was making me take aromatise inhibitors, looking on you tube i found an article talking about micro dosing using test propionate. So i tried it and have never looked back, sometimes these clinic docters have no idea, always pays to do your own research. Great article doc!!

    At 5:55 when discussing the labs, it’s Liquid Chromatography Mass Spectrometry x 2 hence LC-MS/MS. If you are trying to quantify a known compound I hardly see the reason for using ‘MS/MS. This technique is typically used for chemical structure elucidation (determining an unknown chemical entity), not for quantifying a known chemical. With LC-MS there is already a 2-dimensional separation, why LC-MS/MS? Honestly no lab will run MS/MS routinely that’s typically PHD level analysis.

    Thank you so much for your candid and honest approach to educating men about TRT and options that should or could be considered. You are a pioneer for every man who becomes hypogonatic.
    One question about plastics we all encounter daily. It's been suspected that plastics used for food containers introduce estrogens into our diets causing young girls to develop earlier (10-12) than in the past. Do you suspect that the same exposure is causing men to develop hypogonadism in addition to developing prostate cancer?

    on the TOT round table, they were discussing how A.I. were doing more harm than good and are way over prescribed, just like in this video, they talk about how its all dependent on the dose of test given, and a relatively healthy body should aromatize that test at normal levels.

    Hey Doc, thanks for all your great videos! I have a question... there seem to be two thought processes when it comes to running supraphysiologic doses of testosterone and at what level the Estradiol should be. Some say to stick within the range given on our blood work, and some say that it can go much higher than the range, as long as the ratio with testosterone is the same. Can you expect, or is it realistic to have estradiol in the 20s or 30s when total test is > 2000 and free testosterone is over 40?
    Also can you answer how fast do BPH/other prostate related issues arise when on testosterone/TRT. I have been on a self-prescribed TRT for over 6 months and lately I've been having some shooting pains in the perineal area. I've had them before TRT when I was 19, but not leading up to the TRT at 32 yo.
    One last thing: it seems like aromatase inhibitors are a double-edged sword. Whether you take them or not, you are causing some type of damage to the system. By taking them you avoid some of the edema or bloat which put's less strain on the cardiovascular system. By taking them you increase LDL and lower HDL. Given mild steroid cycles (less than 700 mg/week), would the amount of, say, Arimidex used to control estrogen be enough to damage the lipid panel? What is worse, to suffer with edema or poor cholesterol? Or should we aim to be somewhere in the middle... to let estrogen rise above normal range, but not too out of sight to cause edema... and take just enough estrogen inhibitors to avoid most of the bloat, etc?

      The biggest take away is how you feel trumps what blood work says. When you take a large dose an aromatase inhibitor is almost always needed to some extent. Men like woman differ in what is normal or acceptable as far as estrogen levels go. Higher estrogen is preferred to lower estrogen studies confirm this. My advice is if you're doing a so-called blast use an AI but after that get down to a dose your body can handle without aromatizing. Everyone is different. Two men could be on the same exact protocol and one could have twice the level of Test in his blood even though they are on the same dose. Unfortunately, the answer is to drop the dose of Testosterone and AI to prevent both high E2 and complications with aromatase inhibitors.

      @@ryanhust5797 I use masteron and run it as a trt add on. It helps with inhibiting higher estrogen levels and its affects also gives a leaner, harder look to my physique.

    My TRT doc put me on an extra .2mg of anastrozole per week after my estradiol tested at 37.6 pg/mL and I did not feel right after a few weeks so I stopped. Writing this, it just dawned on me that the addition was likely causing me serious joint pain because it disappeared. Sometimes I wonder what I would feel like if there wasn't .5mg compounded with my test.

      Doesn't look like he answered you, so I'm on sustanon 250. 500 a week split into 2 times a week and I started useing Dim and those are 200 ml and I still felt like shit so I take 400 and that seems to work, they're not a blocker but they regulate it 👍👍👍👍

    Hey Doc
    I started my first test only cycle 250mg per week. After 5 weeks I did a blood test and my test lvl was above 15 ng/mL (that number could be above 30 but their method of testing does not go further for reliability of test).
    But more importantly my E2 levels are 146 pg/mL and perfect range would be between 20-30. How concerning is that? Should I start with AI or SERM until the on fo the cycle to reduce that number or should I get of the cycle?
    What do you advise?

      You only need an Al if you feel the e2 symptoms. Some people need Al at e2 of level 60, some can be okay with it even with level of 100.

    Doc is not responding to comments so anyone else feel free to reply to this. It is not clear if you are saying that these problems are being caused by the estrogen being crashed or just taking an AI in general even if it balances your estrogen correctly.

    Doc, Thank you for all the videos you produce. I have a question on micro dosing.. By micro dosing do you mean total testosterone given lets say per week and / or that total split up into smaller doses during that week? To me it seems logical that smaller doses during the week would appear more natural to the body's pulse?

    Arimidex or Tamoxifen, but beware if you've any form of multiple sclerosis, and are on anything other than Copaxone, avoid Arimidex.
    Offset bone density loss with vitamin D and Calcium, even TRT will increase trabecular bone density.
    And if your using Tamoxifen, take a low dose of Asprin, and eat plenty of Berry style fruits, stretch the legs often and add some more light leg exercise to your routine.

      I'm very sensitive to arimidex...even .25 mg will bring my levels down. But if I just don't take the arimidex at all and my estrogen gets high, I will have severe migraines. .25 mg twice a week will usually be enough to keep my e in a good place.

    Estrogen is a CONSTANT pain in the ass for me as someone who is on TRT but also occasionally cycles higher amounts of testosterone in the 500-1000mg/week range. I have never been able to maintain a 500-1000mg/week testosterone dosing schedule for more than 4-6 days without my libido crashing/vanishing. It spikes within the first 2-3 days, then it crashes. This is happening with a DAILY dose schedule of test (daily injections, so no testosterone spikes). This happens whether I take NO arimidex, and it still happens when I take varying amounts of arimidex. I've tried 0.5mg arimidex EOD, I've tried 0.25mg arimidex EOD, I've tried 1mg arimidex EOD, and I've tried 0.25mg arimidex E3D, basically every possible kind of arimidex dosing you could imagine I've tried. Very frustrating, because the gains to me are not worth having zero sex drive, which always returns once I lower the testosterone doseage back down TRT therapeutic levels

      most likely you are better off using a different compund on top of T and lowering the dose of T for cycling purposes.

    If elevated estrogen is causing high blood pressure and water retention, is it better to go on blood pressure meds or AI? I am already dosing 120mg in 3 (40mg each) sub q injections. Total T at 650

    He’s right about micro dosing!! I feel better my levels aren’t up and down and I don’t use any AIs or serms anymore they actually made me feel worse in my opinion and crashed my sex drive like letro, tamoxifen, Clomid etc.. Then again I self medicate lol but Dr O’Connor would be the one Doc I would see for this we don’t have many docs in New England that are knowledgeable about this or that would not judge for that fact!!

    Over all, even if you discount any potential for stimulating growth of preclinical cancers, TRT sounds like the risks increase over long term and AI treatment of any type only postpones complication. If you use AI for a while and then wean off it, what stops the continued TRT from increasing Estrogen at that time?

    I just received my result after the 2 first month of trt 150 mg/week My testo is 1159, estradial :53,87(high?) and SHBG 34, 3. i was 2 month ago Testo 605. estradial 21,2 and SHBG. 49,1 what do you think about my result, shoudl i lower the trt dose? and is the estradial too high?

    I wish I could find someone like you in NY state. I'm about 2 hours from the city and anytime I bring up TRT to local doctors, they get so standoff-ish about it.

    My estrogen was too high, took letrozole and now it's very low, so injecting 700mcg per week of testosterone, how long do I wait roughly before taking letrozole again to stop it going to high? E. G. On this dose will it jump 3, 30, or 300 points per week?

    I'm with my both LH and FSH in very low levels. My doctor diagnosed secondary hypogonadism due to obesity and prescribe the following formula: 100mg Clomiphene + 0,2mg Arimidex + 400mg Tribulus daily stacked with testosterone gel at the night for 2-4 months. Reading online seems too much... What would lead him to prescribe so aggressive dose?

    If a middle aged man is on a high dose of test 500mg/wk and higher if he does not have any symptoms of high estrogen and does not use an AI what are the side effects and possibly long term damage from having high estrogen?

    If you’re going to mention micro dosing you need to give an example on how to do it doc. 16 mgs a day injected with an insulin syringe in the shoulder or quad is optimal. Gets me around 900 total testosterone and 24 free testosterone. Been doing it this way for about 6 months. Never felt better and I’ve been on trt for 15 years. I also do a week of hcg every 6 weeks. I don’t think staying on hcg all the time is a good idea. I tried it and felt great for a week or so, but then started to feel like crap. Not sure if it was estrogen creeping up, but it felt like it.

      Thats 112mg a week, hardly a microdose. If your a genetically gifted freak, you might produce 49mg a week. Your almost 2x the Test, and likely putting more than 2x to use ( in comparison to a natural 1 out of 5000 freak)

    I'm 35, been training since 17.
    191cm 105kg. Did blood work, everything is perfect.
    Want to fo my first cycle ever.. 250mg of sustanon per week, for 12 weeks
    After week 4 I will add 30mg turinabol ED til the end. But I have a problem. Some people say I should take Nolvadex 20mg Ed just to be safe, others say it will ruin my cycle...so I don't know who to trust or what to do. Any...any advice would be much appreciated. Tnx in advance.

    i like how you made a video addressing the side effects and use of AIs and didn't think to even mention or cover Exemstane. Interesting information you're putting out man. Seems like you're pretty biased and trying to argue/persuade one side of things as opposed to educating all with unbiased information.
