    Hey thanks for the explanation. I'm interested in the very first knot you tied. Where you loop the thread around your clamp and then pull the other end. We tried doing that today but couldn't so we resorted to doing a triple reef knot by hand. But as our surgeon showed us, its much faster to use the looping around the clamp and pulling. Can you please maybe show in slow-mo how to do it? That'd be great, thanks. I haven't yet found a video that explains that in detail. We're not doctors; we're neuroscientists who do surgery to implant chronic electrodes in animals.

      Thanks for your comment. The knot is sometimes called an Aberdeen Hitch. You can Google it for more instructions on how to tie it. Perhaps in the future we will feature it in a video ourselves. Of course, it is fine to end this running stitch with a simple instrument tie. But because you are tying one strand to a loop (which has two strands), the knot you form will be composed of three strands. This is a bulkier knot than an Aberdeen hitch by 50%. It may not make a difference, but some surgeons may argue that the bulkier knot has a higher chance of forming a suture abscess/granuloma, or spitting out from the wound.

    Thanks for teaching the running suture. My surgeon left the knot coming out of my wound. I had a temporal artery biopsy . Anyways, now its hurting. I am sure its a running suture. Wound won't close because there is a knot sticking out.

    I used to use this as part of a trychophytic closure but found that there was too much dead space Eventually went to an interrupted vertical mattress suture with a trychophytic superficial closure.

      typically you would use absorbable sutures for this technique! if you use nonabsorbable, i'm sure you'd essentially open up the wound again haha

      As is mentioned at 0:39, fast absorbing sutures such as monocryl / caprosyn (poliglecaprone sutures) are the most common suture used. It is worth noting that you can perform this technique with slight modifications using permanent monofilament such as prolene which does require removal afterward - instead of tying a knot at beginning of the incision you pass the needle through intact skin and out the apex of the incision, perform the suturing as shown, then at the end of the incision pass the needle through the other apex and out intact skin. The suture ends are left long and taped to the skin, the incision often supported by skin adhesive such as dermabond or steri strips. The suture is then removed by cutting one end at the suture at the skin and pulling on the other end.

      This is an absorbable suture type (monocryl) - the skin will break it down and it will not need to be removed.

      absorbable & non absorbable
      we use the absorbable one for this kind of suturing ..

      @@drdipin You can actually, except you dont bury the suture on the upper end, you let the end of the suture outside, kindly traction to level pressure and wound approximation among both ends and knot each it in its own base on the outside. That way, when it needs to be removed, you cut below knot on one side, and kindly traction from the other side. Thats why the method shown in this video has its advantages.

    I would have buried the "anchoring" knot much deeper. Also, the Aberdeen knot at the end is a little more bulky than is necessary, but it's a minor point.

      By burying the first knot I mean that I would have completely inverted the anchoring suture knot rather deeply. Then pass the needle up to the apex of the incision and begin the closure as usual. Braided materials should not be used.

      I don’t know what that is but there are solid rubber ones on eBay what I prefer because you can use dangerous objects to damage the pad such as kitchen knives to creat a realistic wound and then fix it
