It's just luck. Game doesn't have hidden secret ways for alchemy. Each fusing attempt is individual and has it's own success chance, and that chance is not affected by previous attempts. It's like this example: if I flip a coin 5 times consecutively, and get heads every time, what are the chances of getting tails in the 6th throw? Yes, fifty percent.
Well, how do you explain, that we never have two fails after another? After one fail it ALWAYS becomes a success using the next elixir. Thus it is not individual. There is an algorithm and if so, it can be cracked.

Use the lucky pot and lucky powder in the main weapon you are upgrading. Just fail low grade weapon many times then upgrade your main item for +9 and repeat the same process until +12
this could be a video who will make most of you destroy their items and go in a bad feeling mode ... Don`t trust everything u see and don`t believe everything that somebody says just so could be in peace . some of the people do this for views and likes ...
The magic powder and lucky stone will help to have more chances of success. Fail item first but dont put lucky stone or magic powder on those fail items but put it on your main upgradable item. It's a trick, because this game doesn't have hack or cheat code to easily upgrade items.

I apply this philosophy, fail other item many times to get a higher chance on upgrading the weapon successully
