Yes we always use V3 that is third form of verb in passive voice and there is no form of verb allowed without v3. Hope your curiosity is nomore and hope you have got my point thank you.

First one is a simple sentence so it doesn't need an explanation and in the second "does" is used because we put emphasis on it matlab hamnay is baat par zoor diya hai ki iskay paas *hai* migraine ki history. Hope you have understood if there is any doubt feel free to ask
