    Dr. Towfigh offers a range of techniques, and from time to time she interviews some surgeons who do the same thing: they chose the technique that will best suit the patient's hernia.
    Glad to hear it.
    In Manhattan, NYC, I can tell you that (almost) every surgeon is skilled in ONE technique (and here I'm referring to inguinal hernias) and that's all they offer. It's either Lichtenstein (open repair with mesh) or Lap (and here it is almost always TAP because it's easier; finding a surgeon who even does TEP -- Brian Jacobs, for example -- is rare).
    Shouldice (or any kind of suturing / non-mesh repair) is almost impossible to find here. And let's be frank: that's because there's no money in it. Surgeons want to operate. But they also want money. And a suture / tissue repair pays A LOT LESS than a Lap TAP (or TEP) or Robotic procedure. Suture repair? No mesh? Good luck even finding that in Manhattan.
    NYC surgeons do ONE type of procedure. Mostly Lichtenstein or Lap TAP. And they don't care what you present with. It's their way or the highway. You can either get on board with the one technique they offer, or you can get lost.
    And what do we have to do: Go to Dr. Kang in Seoul, Korea just to get his modified Bassini suture / no mesh procedure?? Or is he one of those Unskilled and Unaware surgeons in Asia that Dr. Towfigh starts talking about @ 2:30 in this video??

      I’m sorry to burst your bubble, but for the surgeon, open repairs pay slightly more than laparoscopic or robotic repairs. And, the use of mesh or not does not affect the reimbursement. This is based on standards of relative value units determined by Us government, CMS (Medicare).

    Just had robotic laparoscopic umbilical hernia repair 2 weeks ago and the sensory nerve damage you described regarding extreme pain when barely touching the skin over top ports. There is also a section of numbness below this. The surgeon abandoned me, refused to prescribe pain meds and accused me of drug seeking!! Not true. This is real pain at 10 level. Utter negligence. Yes, the nerve block idea might help. Thank you for your great info.

    Dear Dr. Towfigh, I had a small umbilical hernia repaired with a couple stitches laproscopically. Have you ever heard of this? I am doing fine but a little pain in the umbilical area. Also, adhesions were taken down and placement of seprafilm was used. My repair was yesterday and I plan to go home next Tuesday. Hopefully everything will be o.k.

      Small umbilical hernias can be repaired without mesh.

    I know you probably won’t answer but here’s my question. I live in New Jersey and just had robotic laparoscopic surgery with mesh using the Da Vinci Machine this past Monday 6/3 for a spigelian hernia. Nausea, shoulder pain, headache, throat phlegm from the breathing tube is all gone. Was constipated but had 4 bowel movements yesterday after taking miralax and a stool softener. I have 4 small incisions that all seem to be healing and no pain near incisions. My question is I feel so much pain & discomfort where the repair was done especially when walking and getting up. Feels like it stabs, burns, tugs & aches in that area & groin and pain when standing in my side/hip. I’m so scared that this might not be normal. It almost feels like nerve pain. Should I be feeling this after 6 days? All vitals are good too. Appetite is decent too.
    Is this normal pain expected?

      Pain after 6 days of major surgery is not unexpected. Follow up with your surgeon for detailed questions about your operation

    Hi I had hernia double surgery five months ago and since I have testicular pain to the touch. Also if I lift anything over 10 lbs I get pain in groin area . My groin isn’t sore to the touch like my testicles are. Any thoughts on what could cause my pain. Thanks

      Can be related to the interaction of the mesh with the spermatic cord. Groin pain can also be related to unstable repair with billowing of mesh into the defect.
      It’s all treatable.

    I think i have this 2 weeks after hernia repair. I was sitting in car and then pain came up testical hurt like someone wacked it wirh sledge hammer. I always get like electric shock doen my leg from my thigh. Then i get like tingling when my supported boxers brush across my thigh. The top part of thigh is tinkging all the time. Like pins and kneedled. Sergeon said i could suffer chronic pain from the operation. I didnt realise it would be like this. Most the time im fine but then it comes and goes comes and goes. Its weird. Its not disabling get i can get past it but it defo dosent feel normal. Maybe the branch will grow back maybe i need to sit and wait to get checked watch wait and see i dont know. The area around scrotum still feels tender and bruised. My left testicle seems like its sitting further up instead of rifht testicle what hangs more. I dont know what they have done. But it dont feel rifht most videos i see 2 weeks post opp are all having a decent recovery. Mine hasnt been so straight forward 😢 you need to come to UK and check me out 😢😢
