      A well fitting denture causes less lisp. so go to a good dentist. The material won't have much impact on the speech itself.

    Hi doctor. My orthodontist extracted 2 upper wisdom rooth. One was healthy and straight. But second was still under gums. He extracted both at the same time. He said i have overbite and i need to extract them. And after 6 month i seethat my cheeks become flatter than before. Is that because of extractions?

      Hi there. Wisdom teeth hardly help with maintaining the position of your cheeks. Even losing 2nd molars won't really impact your cheek position that much. However, if you were to lose multiple back teeth, then yes, your cheeks would start to collapse and it would be quite visible. I think you are overthinking the scenario.

      @johnjazayeri I have 14 teeth at the top. Yeah, probably I' m overthinking, but I asked 10 people if my cheeks changed. And 8 people saw the difference.

    it destroys your face that's what it does, idk why orthodontists still extract teeth in 2024 when you can just expand. It ruined my face, I have a receding chin and will only get worse with time. Once I get my braces off. my teeth will be retracted and so will my face

      Extracting teeth is can be a tricky situation. It's not always a bad thing either. You can expand some cases, but that is difficult and painful. It all comes down to how crowded your teeth are in your jaws with regard to your jaw relation. I'm sorry that you feel your orthodontist mistreated you. However, you can't just assume that no one should extract their teeth. There are still many cases of patients with teeth crowding that require teeth extractions.

      @@johnjazayeri I'm not sure. I have an overjet and minimal bottom crowding, which there is only one tooth overlapping another. I think I was able to use an expander to create space instead.

    Hi John, thanks for the video! Im being told that I need a root canal. However, both my parents had root canals that got infected. This made me do some research, and i keep hearing story after story of problems and infections. Im paranoid about getting one. Can i just go straight to getting an implant as they generally last so much longer and have far less problems(at least from what ive seen)? I guess the question is, why is there such an emphasis on "saving the tooth?" Hope to hear from you soon as i need to decide soon. Thanks for any help!

      Hi Dave. First, make sure you really need a root canal. Is the tooth infected? Is it damaged from a filling or a crown? There are cases where a root canal is a viable option. I would consider root canals for: 1. Front teeth 2. Trauma pain, not infection 3. Younger patient If you meet these criteria, then a root canal is not a bad option. Also, many people go for the root canal because their insurance covers it and it doesn't cover dental implants. Also, note that 70-80% of root canals fail in 20 years and become dental implants. This is why many people choose to skip the root canal and go straight to the next step. As you can see, it's a case-by-case situation. The more knowledge you have, the better the decision you can make. What is your situation?

      @johnjazayeri Hi John, so the tooth in question had sensitivity to cold and air. The dentist said it looked to be from a receeding gum(it looked slightly high to my eye but not too much ). He put a filling over the receeding part, and it seemed to help the sensitivity a lot. A week later, however, I woke up with a full toothache. For about 5 days after that, it was an on and off ache and very sore to chew on. Suddenly, on the day i was booked to see another dentist for another opinion, the tooth basically stopped hurting. She said the tooth was dead and the pain would come back. Does this sound right? I was hoping things had corrected themselves, haha. She did a cold test, and i didnt feel it. Since then, the last 4 days, it's basically been fine. Slightly sore here and there but hardly noticeable. My gut leans to implant(if either are absolutely necessary) but still unsure what to do. Thanks for your reply! :)

      Hi there. I would say, yes, the second molars are the least significant teeth to replace with dental implants. You really don't need these teeth to chew well. The main thing that a dental implant does in this scenario is to keep the opposing tooth from shifting into the space. So if you were to lose #2 and not place a dental implant, #31 would gradually move up. This happens over the course of years, decades in fact. Could be a bit of an issue if you are younger. Hope this helps you decide on what to do. Good luck!

    Well, my metal partial doesnt look like that. It looks like a whole upper dental plate. Way too much metal...way too tight...very uncomfortable. Im seriously considering the flexible partial. I have a consultation soon so I hope it will work out good for me ❤

      Thanks for sharing. Metal partials get looser after you wear them for a while. Several weeks to a few months I would say. If they don't, you can ask your dentist to loosen the clasps. If none of that works, then you should explore flexible dentures or dental implants. Good luck!

      Hi Marion. Oh boy, don't just pull two teeth because they have broken fillings. Can you put a crown without a root canal? That would be my first choice. A denture may seem like an easy way out, but it would cost you more long-term, plus it damages the neighboring teeth. Let me know what you decide to do. Good luck!

      Hi Marc. Sorry about your dilemma! I would focus on finding a good, caring dentist who can help you transition to dentures. You can always add dental implants in the future if circumstances change. Think of it as a step by step process. Besides, some people do very well with dentures.       Hi there. Great question. Technically, dental implants can not get an infection the same way natural teeth do. This is because cavity bugs are not interested in foreign objects, which is what a dental implant is. Now, dental implants can still fail. Technically, this is not an actual infection but more like failure to integrate. Also, you do not need to worry about cavities with dental implants. Does that mean you shouldn't clean implants? No, because you still need to maintain the jawbone. However, dental implants do not get cavities like natural teeth do.

      Hi there. You are correct, dental implants are less dangerous than root canals because they don't get infected. I have a few patients in a similar position. You can start off by removing the infected teeth on one side of your mouth and replacing them with dental implants. Then, the following year, you can treat the opposite side. This way you can continue chewing comfortably while also having time to pay for your treatment. However, I wouldn't leave infected teeth in my mouth due to a fear of being unable to chew. The consequences of chewing less effectively is far less than leaving infected teeth in your mouth.

      Hi Tesa. Slight misunderstanding here. There is minimal facial support with dentures. All-on-4, and permanent implants, have more facial support. Natural teeth are the best! The reason is because dentures are mobile so they are not strong enough to keep your facial muscles where they need to be. Dentures help somewhat, but they are not as effective as dental implants. In other words, you will develop more wrinkles without dentures then you would with dentures. Similarly, you would develop far fewer wrinkles with dental implants and all on four then you would with just dentures.

    If I decided to do bone graft after 2 weeks after front 1 teeth extraction, will it be more difficult or complicated? I didn’t know or I was not advice in advance at my dentist when I had an extraction! I’m from Sydney, Australia 🇦🇺 I plan to have an implant but at the moment because of a family funeral they put a temporary denture at the moment 😢

      Hi Bernadette. My condolences to you. You can't do the bone graft after treatment. It must be done at the time of tooth extraction. Does that mean you can't place a dental implant? Absolutely not. You will have less bone to worth with, but an experienced implant dentist can still place a dental implant. Even though it might be more difficult and require a bit more work. Wait 3 to4 months for the area to heal and consult an implant specialist to see how the area looks and what the next step should be. Probably best to consult a different dentist since the one that removed your tooth did not care to preserve your jawbone. Good luck with your treatment!

      @@johnjazayeri thank you John 🙏 I did saw the specialist who does the implant for my front teeth, she said to wait 6 months to get my gums to settle and bone to grow ? She also said that she would put bone graft if needed when she starts the implant if needed to ! She wants to do more implant on some bottom tooth that I lost many years ago , I asked how is the bone loss in them , she’s stated they are okay 👍 so I’m not sure 🤔 I might get a second opinion! My husband, says do one implant which was the front tooth which I have a denture at the moment! If I’m happy with her work, get all the others done too ! What do you think? Should I get another professional dentist Doctor opinions ??? Thank for responding to me !     Thank you for this video Dr. Jazayeri. I truly appreciate it. I am going to my 2nd Oral Surgeon tomorrow and ask him to take out a back molar that I had a root canal in by a traditional dentist. As soon as I had the root canal I got tongue blistering terrible to the point that over several weeks I ended up with many symptoms putting me in the ER. Of course ER drs didn't know what they were looking for or at. So the tongue pain was horrendous. That has subsided and one side of blisters is gone, but I don't trust this healing process. I want the tooth out. The tooth itself started to hurt where the root canal was done so I know something is up. Truly I will never have a root canal again. I had perfect health until I had the root canal. I have no idea if he will take it out but I will find someone who will. I would rather have a bridge or go without it as it is in the back.       Hi Tammy. I would get a "space maintainer" for your child. You may have to go to an orthodontist or pediatric dentist as most general dentists don't perform this treatment. A space maintainer is a small metal extension that keeps the gap open and preserves neighboring teeth from collapsing into the space. This is a big problem for young developing children as the teeth will shift into the gap and create a big problem. It's much easier and cheaper to do this than to go through braces treatment. Also, you can get the dental implant sooner if you decided. I would say by 17, when the jawbone is fused, is a good age to start implant treatment. Good luck!

    Thank you for the info. I have metal dentures and my dentist told me that when I remove them at night I should let them dry during the night and not put them in water. Do you have an explanation for why he said this. I am a little confused because I have always put them in water.

      Put them in water. Leaving them dry causes the resin to shrink and crack.

      Hi there. Well, 2 years is not really a baby. Would I let a dentist with 2 years place a crown on me? No way! There are so many subtle things about a well-fitting crown. Getting the perfect bite, contact, shade, margins, size, material, etc. requires lots of experience. Maybe you should ask him how many crowns he or she has performed to get a better idea. Personally, I would prefer someone with 5 years of more. Someone in their thirties at least. Good luck!

      Pulp caps are a hit and miss. I definitely prefer them a full on a root canal. However, if the cavity bugs have infiltrated the nerve, a pulp cap usually doesn't work long term. It's certainly worth a try if the tooth is not infected and the pain is minimal.

    The thing is my tooth is not infected. I had a filling which came out and some coconut got stuck in there causing a gum infection but my dentist said i need a root canal when I only wanted a filling after I did a course of antibiotics. In my appointment I will tell her that i dont want a root canal as it can cause infection unnoticed due to the dead nerve!

      There are many factors to consider here. Personally, I would only recommend root canals for teeth with trauma ,like fractured teeth, pain after receiving a filling/crown, etc. I would avoid root canals on teeth that are already infected. The worse the infection, the lower the chances of a root canal resolving the infection. Similarly, a chronic infection responds much more poorly to a root canal treatment as opposed to an acute scenario. Each case is different and you need to evaluate factors such as source of pain, condition of tooth, remaining tooth structure, etc. to make the right decision.

    Very informative video, thanks so much! I had implant on one of my right up tooth (#4) in 2015, but the implant was taken out in March of this year because it got loosened probably duo to the bones loss on that area, the dentist put bone graft in the same time during take out my old implant for considering future's new implant application. Unfortunately, today during the procedure for putting a new implant, the dentist stopped in the middle of procedure ( even the my gum is cut open already), he told me my bone is too thin to put in, it is better for me to have a periodontist to do more bone graft first in that area. I feel sad and frustrated, I really don't want to have another bone graft procedure again. I think the bone area of my lost tooth (#4) maybe not suitable and not good spot for implant anymore. I also don't want to have permanent damage for my other two healthy teeth for bridge. After watching your very detailed video, I am considering to do flex single partial denture for that tooth. I hope you can see my message and I am looking forward to your professional advice or suggestions for that. Thank you in advance.

      Hi there. Sorry about your dilemma. This is a tough situation. It is very rare for a dental implant to fail after ten years. Are you sure it wasn't just the abutment or the crown that was loose? 99% of times that is the case. Anyways, too late for you. Once you lose a ten-year old dental implant, there is very little jawbone left behind. You will likely have to restore the jawbone prior to placing a dental implant. The area around #4 has poor quality bone to begin with. It's not uncommon to have to spend 6-months to a year to grow the jawbone prior to placing the dental implant. A sinus lift would be the alternative solution. I would avoid a bridge at all cost. Keep me posted. Best of luck!

Actually the crown was changed to a new one in the dentist office when the first time I noted the original crown was loose last year. It is just like you explained, I can clearly see from the X-ray of the jawbone line of my #4 jawbone is much higher (above) than the jawbone lines of the two side healthy teeth due to the bone loss in that area. ( maybe it was due to my inexperience and ignorance or financial concerns etc after that #4 tooth extraction, I didn't see dentist for consulting an implant immediately or sooner, the implant was put about 2 to 3 years after the tooth extraction, it probably have some bones loss before applying the implant). And still continued having more or less bone loss even after an applied implant in the past ten years. That is why my dentist stopped in the middle of putting a new implant procedure this time and told me that I need more bone graft on that area for a successful and effective implant, he is a good dentist, I trust his judgement and believe he thought from the best benefits from my side for my specific condition. But anyway, I already feel tired of putting an implant on that tooth and afraid of the oral procedure of the bone graft (cutting open the gum and putting stitches suture etc), I will just do a removable denture for that tooth alone to spare the pain and suffering of invasive dental procedure. Once again, thank you very much for your great video, it gives me very helpful information and confidence for an alternative choice of that missing tooth, to consider a flexible denture for it.     *PLEASE* *HELP* I have a few questions how long does it take because I know they have to extract the bad teeth and wait for gums to heal so from the first visit that you got the work done until the last visit that you had the work done how long was it? I need like 10 molars replaced top and bottom I crushed my nose and broke quite a few teeth in a bad car accident and that was 15 years ago I’m 40 today and I’m worried it’s gave me some sort of heart disease although I’m relatively healthy and look fine but looks can be deceiving we all know that I think it could just be my anxiety tbh. Today they gave me 3 options today at the Dental office 1.) is go the implant route it’ll probably take a year to get everything finished and it cost $28k 2.) Do implants on the top and partials on the bottom 17k or 3.) just get the bad teeth extracted and do partials because it’s a lot more in my budget and for that it’s only $5,500 and they could extract the teeth in 3 weeks and have the partials in by Thanksgiving this year (2024) and said I can always upgrade to implants later when my money gets back up again. I don’t have any dental insurance so this would be all out of pocket and they’re recommending Proceed Financial. I just don’t know which to choose I have good credit so I could probably get approved for the implants but that’s another big bill every month I’m 40 recently single and depresssd and I’ve always been the life of the party bachelor living Las Vegas dating beautiful woman but right now I’m depressed and down in the dumps. I wish I knew the right answer here,smh?? **If I did get the partials I definitely would like the ones that don’t show the wire is that always an option?

    ***Every time you put food in your mouth, every time, and every time you put any drink in your mouth except water, everytime - wash your mouth out good with water afterwards. If you are not in the process of eating or drinking, there is no reason to have any bits of food or drink in your mouth or gums, etc., , at all. Start doing it. There is almost always a sink around you can get to within a matter of minutes. Always keep your mouth clean. And brush your teeth good twice a day, floss once. Good bye any dental problems. I am age 60. Have all my 32 teeth; nice and white and strong, hesalthy. They should teach this in homes and even in schools. So easy to do. I have been doing it for over 30 years. Finish eating? I go o a sink and wash mouth out, clean mouth. Drink a can of juice or soda, etc. Get to a sink pretty much as soon as possible. Same thing everytime you eat or drink. Everytime you put anything but water in your mouth. I also have not seen a dentist in over 20 years. Keep your own mouth and teeth clean. I will happilly see a dentist, don't feel I need it now, though. Seriously, every time you eat or drink, afterwards wash your mouth and teeth, gums good with water. You will walk around with a nice feeling clean mouth all the time. I don't even get it? People eat and then simply go about there business without rinisng mouth out good with water. Why? Then if they drink a can of soda. so? Just go about my business. NO! Wash out your mouth good, rinse good, with water after you finish the soda. Etc..Always keep the inside of your mouth teeth, gums, etc. clean by rinsing good with water. You will thank me as you begin to realize your mouth and teeth have way less problems, likely none at all now. You finish drinking a can of soda or juice or iced tea, etc. Why the heck don't you just go to a sink and wash your mouth out good with water? Keep teeth clean and white, Gums healthy and strong. Start doing it.

    Hi Dr. J, fellow dentist here 33 yrs in the field. I'm curious as to why you say case 3 can't have an RCT. Sure it would be challenging. But I see no reason why a very well instrumented and obturated RCT wouldn't work in that case. I didn't see radiolucency on the side of the root so vertical fracture is unlikely, just pulpal necrosis.

    Thank you for making this! I was told 2 days ago I needed a root canal on tooth 29 due to a failed filling/trauma, so naturally I started doing my own research. Saw the documentary "root cause" and was scared to death and started considering going straight to an implant from a wholistic dentist. Once I found out the wholistic dentist does root canals too I started to reconsider, and now I'm sure I'll just do the root canal. I was wondering if the lower teeth are less risky, and since its only a bicuspid my dentist said he would do it himself, should I insist on going to an endodontist? And do I need to demand the CT scan at least 1 time a year, or should my dentist already know?

    Just had this procedure. The extraction is mostly paid by the dental insurance, but not the bone graft with membrane. My bone graft is related to 2 charges: D7953, Bone Replacement Graft for Ridge Preservation, and D7956, Guided tissue regeneration for edentulous area, resorbable membrane. Both are not paid by my dental insurance (they changed D7956 to be paid by the customer in 2023). D7953 costs me $450, and the stupid membrane (D7956) costs me $890. I actually found another membrane code D4266, which is covered by the insurance, but it is for natural tooth site, not edentulous site. The membrane was kept there by a couple of sutures and by second day, it has already fallen out a bit at one side. I understand the membrane is supposed to keep gum from growing into the bone, but I think it is a joke that it costs $890!!!

      Hi Terri. You are correct, a lot of professionals are arrogant. Personally, I am not a fan of Fluoride, Amalgam, nor root canals. But what's wrong with dental implants or dentures? I mean if you lose your teeth, you have to replace them with something. No one complains about an artificial hip or knee when they have to get a replacement. The same goes for teeth!     you are awesome...i wish you were in cleveland, i would trust you..... i had my bottom denture and top partial for 10 yrs..... i have 5 top teeth.... i broke the bottom denture and wanted to replace both. i have 2 front not the main 2 front but one main and one next to it then skip 2 on the other side then there is 1....i wanted to have the 3 taken out and a new top partial so it can look really nice with the new bottom i want.....well the dentist the 3 teeth are good teeth and i should keep them and they wanted to give me a temp partial and i refused so they want to say i need an oral surgeon for extractions which would cost thousands more and i had already paid down payment and payment plan..... they didnt say that until i changed my mind about the temporary partial... i just wanted extractions of 3 teeth....top permanent partial once healed and a full bottom.... at that point i felt like they were trying to rip me off....anyway, im very depressed about it, no insurance and low self esteem and absolutely no confidence in myself.....i hate taking pictures..... i just want a nice smile

      The rest of my teeth were pulled in March. So I haven’t had any teeth in six months. It does really make you feel bad. About yourself and I don’t want my picture taken. It is ridiculous that the dentist is being like this. After you paid all of this money. I am so sorry.💔😢
