Idk if this will help at all, but I've enjoying Ben Patrick's work for taking care of your body from the ground up and focusing on strength through mobility, starting at the ankles, knees, hips to shoulders. He's known as the Knees Over Toes Guy. A quick summary, he had knee injuries and surgeries, was told he wouldn't regain full health again but rehabbed himself off of pain meds, regained complete mobility and increased his vertical from 19" to around 40" to be able to dunk. Perhaps there is something there you could help rehab or create better mind-muscle connection in those target areas for you. Best wishes.
