    My experience is very similar.I was diagnosed in 2002 age 50.Psa 11.5.I had surgery and Gleason was 8 and positive margin.PSA after surgery was 0.03.After 2 years my PSA had risen to 0.2 and I had 30 zaps of radiation.PSA then dropped to < 0.03 ( the lowest measure at the lab) and that is where it has been for the past 21 years!!.I still have annual PSA checks.

    Great post Neil. Hoping your next PSA is <0.01 mate. I had blood test on Wed and waiting on result. 0.25 at start of SRT to 0.09, then 0.11 so hoping for drop again but in all likelihood it may rise slightly so not out of the woods yet 😊

    Can I ask why you are not encouraging men to get there PSA testing from there GP a PSA test that’s free. I went to my GP and got mine and my Psa was raised. I understand if you can afford to pay that’s fine, but there will be some men who can’t pay. Great video 😊

      @@daveyoung2056 many thanks dave, I have a lot of friends here in Norwich who have done this and got refused as had no symptoms. Going to the toilet at 3am apparently isn't really a symptom. There is a program out soon saying doctors are refusing to do a PSA now. I'm not sure where u are from but here in the UK it will be on the news soon.

      Hi, if you are a black male 45 in the uk or a white male 50 or over it is your right to have a Psa test and your GP cannot refuse it, this is fact and this information comes directly from my consultant, if anyone gets refused they should push back hard .I live in St Neots

      Cheers Dave, I beg to differ, I have many people who have gone to their local GP and asked for one because they are my age (EARLY 50'S). They were told they weren't going to be tested because not enough symptoms to warrant this. If you want a test you have to blag and tell them you go 1am 3am 5am, pain etc. I spoke with a trustee from this Graham Fulford Prostate Foundation that there will be a news story very soon saying doctors are refusing to do the PSA. I agree with the statement saying its your right but in real life it doesn't happen. The NHS don't want to promote that they don't test unless you have real issues. I will keep you posted about the news story about to break in the media when it happens. I def agree promoting going to your local doctors but I was trying to promote a different way to when you get denied this test for no reason. I didn't want men saying "ahh stuff it then i won't bother" when there is a different way going privately and cheaply. St Neots not so far away then, I'm south of the county of Norfolk and skittle past yours on the way to Melton Mowbray now and then. Cheers Mate.

      I had a PSA test as part of a bi annual BUPA medical 23 years ago.I had never heard of a PSA.It came back 9.I rang my doctor in Stowmarket and he said ' what did you do a bloody stupid thing like that'.He went on to say 'you are too young to have cancer'.Of course he was wrong but does show the attitude of some doctors

    Thanks for the update, Neil. I also have 3+4 with a "mild extension", and I've opted for radiation therapy instead of a radical prostatectomy. I made that decision based on my research showing higher risks of potential long-term side effects associated with surgery. I'm glad to hear you've recovered well from yours. You are 100% correct about having people to talk to. Every sharing in a UA-cam comment has value. I'm not worried about my mortality, but there is still something going on in the back of my head making me extremely anxious. My subconscious is probably more at odds with the side-effects of all the therapies than anything else. Plus, the prostate cancer journey is a long one since it is considered "slow growing", and all the waiting involved also takes its mental toll, I expect. I start radiation in about 6 weeks and on my birthday no less. Party time! 😀

    Thanks for posting Neil , I’m now just over four weeks since my operation and starting to feel like my old self, it hasn’t been a breeze with an internal stomach infection and the nagging pain in my shoulders, neck and arms from the gas but that has finally subsided . Your videos have been a great help to me and I’m sure to many others in the same boat , thanks once again Neil

    I just turned 62 and was just diagnosed Jan 2 2025 after had Prostrate Biopsy because PSA tripled after being 3 every year. Still deciding between Radiation or Removal. I'm leaning to Radiation but Doctor, family and friends want me to do removal.

    Neil - just seen this. A lot of people suffer from Seasonal Affective Disorder (SAD) - not just PCa patients. Would a SAD light be of help? I am not a medical person, but anecdotally a lot of people find them helpful. Hope the hernia op goes well, and you get it sorted soon. Sorry to read of an extra trouble to hit you.

      Cheers mate for your comment. Still no news on date for Hernia. As for the SAD light you could be right but i have never suffered from this. All seems fine now. I just come back from Tenerife so the vitamin D was welcome. Going again in March which is just round the corner. Again thanks for watching and taking the time to comment

    Hi Neil I also live in norfolk and also what you was diagnosed is the same as me I was 8 weeks before you February 2023 same surgeon Lille also same reacurance 18 months later. My question is. After 0.1. It raised 0.3 then 0.4 pet scan no show at 0.4. Was told to wait until it raised again 3 months later it’s now 0.6. But they want it to go to 0.8 before my next pet scan as nothing showed up at 0.4. My next pet scan is April booked. You have had your radio treatment now. So did it show up on your pet scan or have they guessed where they think it was. Thanks Alan

    Deciding to go ahead with prostatectomy can be mind a bending and decision to take in low grade cancer diagnosis. There is a school of thoughts saying that Gleason score of 3+3= 6 shouldn't even be considered as cancer and therefore not treated. They more and more recommend the active surveillance in such cases. Of course one should realise that even targeted biopsies can miss cancerous tissues and return a negative results. The evolution of the cancer vs age is a key too. Personally I was diagnosed with a Gleason 3+3=6 in two different biopsies(the 1st was targeted and the second targeted + random) done 3.5 years apart. The difference was that the second time around one of the suspicious areas went from 1 cm . to 2 x 3 cm in size. One urologist said let's remove that prostate whilst the conditions are good. I got a 2nd opinion from a university hospital which confirmed the recommendation. However they said there was no rush and I could wait for another 6 months and do another MRI. Possibly another biopsy. I wasn't too keen on the latter as it can jeopardise the success of the surgery. After careful consideration and tons of research(already mostly done after the initial diagnostic) I decided to go ahead with the surgery. One important statistical parameter I took under consideration was that, in 40% of cases the cancer turns out to be of a higher grade or stage after the histology examination. I was one of the contributors of that 40% statistical risk as some of the cancerous tissues were Gleason 3+4 = 7 after dissection and inspection under the microscope. I feel comfortable that I followed the recommendation of the (very experienced) surgeon and took the appropriate decision. First post op PSA dosage is in 3 weeks.

    Diagnosed with Gleason 3+3 = 6, Grade 1, prostate cancer in November. Discovered in pathology after HOLEP laser surgery to reduce the size of my prostate. Mayo Clinic recommends Active Surveillance for my type/grade of cancer. Will have another PSA test and 3 Tesla MRI of prostate next month to make sure there is nothing else in there that might need more active treatment. Not my first cancer. Treated for Hodgkin Lymphoma in 2020/2021. Chemo and proton beam radiation. Just passed the four year mark with that one. Complete remission. Life has a way of throwing more and more stuff at you as you age. LOL I've also had partial thyroidectomy, double cataract surgery, quadruple hernia surgery, spinal surgery, ablation surgery to remove pre-cancerous Barrett's tissue in lower esophagus, and numerous (at least 25) upper and lower endoscopies over the years. Oh, also throw in a broken foot. I just keep plugging along at age 77. I made up my mind to just do what I need to do to stay alive as long as I still have quality of life. You have to look out for yourself as Mother Nature could not care less about you. 😁

      John, I'm certainly no doctor. The PSA is the best test out there but it's certainly not reliable. I know people who have a PSA of 7 or 8 and they have an enlarged prostate which produces more Psa. It may be an infection. There's loads of different things it could be other than cancer. Go with your GP's advice. You will prob have an MRI scan then maybe some biopsies depending on ure MRI results. Every country do things different but here in the UK I think it's psa test, then digital rectum examination then MRI then biopsy. Stick in there bud and please keep in touch and I will try guide you the best I can. Thx for watching my video.

    I’m in Norfolk and had my prostate removed in November 24 Nov in the N&N, horrendous urinary incontinence. I had the follow up PSA test the other week and am seeing the surgeon at the end of the month. The surgeon I had sounds very similar to the one that you spoke about. What is his name, was he American? I’m not happy about my one and will be challenging him about his antics when I see him.

      Hi Gary mine was Mr Ilie. He is Romanian. I believe yours could be Mr Knight. I think it all depends if they have to trim back your incontinence nerve. Did you have a positive margin?

      @@garypritchard4369 ahh the doctor was in oncology not urology. Not sure of his name but think from Sri Lanka. It was the 1st time that I had seen him. A positive margin is when a tumour is touching the outer of your prostate when they dissect pathology wise. It may touch some of the nerves that are attached. They try to trim up when doing the operation but trying not to take to much.

      @ My surgeon is going to tell me the biopsy and follow up PSA results at the end of the month. He seemed quite positive after the surgery that the cancer was still isolated in the prostate. I won’t build my hopes up until I’ve spoken to him.

    I've had a few "mini meltdowns" myself over the past month or so. Generally, I'm a very pragmatic person, and I felt like I was taking it all in stride when I was diagnosed with 3+4 in the middle of 2024. I'll be starting radiotherapy in around 6 weeks combined with a testosterone blocker for a number of months. It is easy to intellectually understand that your prognosis is very good, but the closer the time for treatment comes, the knowledge of all possible significant side effects, and just the waiting adds up like a little time bomb in your subconscious. One thing doctors need to emphasize more, especially for those of us with lower Gleason scores, is that 1) prostate cancer is slow-growing, 2) treatment will likely be a slow journey. (For reference, I live in the US, so I don't know how my experience aligns with those in the UK.) 1) Blood test: Hmm, we need to look a little closer 2) Blood test: These numbers are concerning, and you might have prostate cancer 3) MRI test: Yep, you have prostate cancer 4) Biopsies: This is where the cancer appears to be in your prostate (no biopsy is 100%), and this is how much you appear to have 5) PET scan: The cancer has/has not metastasized (spread elsewhere) 6) Discuss treatment options 7) Start getting treatments scheduled The above took around 6 months (given, I was out of the country for 4 weeks of that, so that impacted the timeline somewhat). I think #6, treatment options, probably has the biggest psychological impact. Those with Gleason 6 (Stage 1) don't need to do anything, and that's great. Those with Gleason 9/10 (Stages 4/5) need to throw everything medical science has at their cancer ASAP. Gleason 7 (Stage 2) is this weird grey area that is further subdivided based on multiple factors. You have to decide if you are going to have your prostate removed, if you are going to leave it in and get radiotherapy, if you are going to take hormone blockers. There are many other treatment options, some that might sound like a better fit, but aren't available either because or where you live or what your insurance covers. If you decide, for example, to go with radiotherapy, are you going to get a hydrogel insert to potentially reduce side effects? Will you opt for short-course or long-course radiation (assuming you have the options)? What genetic tests to take to guide your decision-making... and then with radiation, it can take up to 2 years for the most drastic side effects to even manifest So, even if everything looks good immediately after the procedure, there are 2 years of waiting to see what is going to keep working right down there and what isn't... TL;DR: Even if you believe you're doing fine after your diagnosis, you *will* have an emotional reaction. You *will* have some very bad days. And as you said, Neil, it is important to talk it out. I would urge people to, yes, share with your significant other, but don't make them your sole outlet. Remember, they are going through a rough time with your diagnosis as well. Seek out support groups, clergy, friends. Thanks for sharing your journey, Neil. I'll follow along, because I expect my journey will be quite similar to yours... except for the Tenerife part. I'm a little jealous of that as it sounds lovely while I stare at the snow outside my window.

      Hi Neil had my op on New Year’s Eve and recovering at home now, everything going fine with catheter and shoulder pain easing but yesterday I have developed severe abdominal pain coming from one of the incision on my left, finding it difficult when I walk , spoke to my GP and a stronger pain relief was prescribed with antibiotics, did you have any issues like that

      @barryhill363 hi there Barry, thanks for watching. To be honest yes I did. They found I had an infection in one of the wounds. I had to go on strong anti biotics for it, but I didn't have no problem in walking. Certainly keep an eye on it. Will try to find out what your could be. Make sure you keep drinking plenty of fluids.       Your just a few days after your op Barry, the healing process goes like a turtle in many of these cases, I started to get back to normal after 6 months and now 9 months later am completely recovered with no more side effects. Take it easy, drink lots of water, don't over extend yourself, beyond what your body feels is good and slowly but surely you will heal and get back to yourself. Time is the best healer. And of course consult your physician if you think that something is not quite right. Good luck and god bless!

      Thanks Neil, you're definitely making a positive impact with these videos. Love the English countryside too . Although they call my state the Garden ( New Jersey) State it doesn't compare to the U.K. p.s. I'm 3 months past radiation and currently on ADT, UGHHHHHH.

    After 37 sweeps of my prostate bed this year plus now one year of depactyl injections in the bum plus another year of them to come after my first review 3 months after my radio finished I’m at an undetectable less the 0.01% on my PSA ,, this was done in Colchester hospital Essex ,, I know things can change ,, but for now I’m very pleased with my treatments

      @@retiredpainter259 great news mate, I have friends down in Essex and speak highly of the hospital. I hope all is under control for now. My psa is 0.04 and hopefully will do another test in the new Yr. No hormone therapy for me as yet. Hopefully I won't need to. Have a great Xmas mate and appreciate you finding my videos.

    Hi Neil, I just found your channel. I had my prostate out at the Norfolk & Norwich hospital in May 2010 aged 55 ever since then I get my PSA level checked every 12 months and its always been 0.01 just had done again in Oct its now 0.1. I rang our surgery to see if I could have another one, they were reluctant to do it but I insisted so I,m having it done again on 23 Dec so fingers crossed. Thanks for sharing your story.

    Best wishes on your journey Neil. Hi from Texas USA. The thinking about prostrate cancer is changing. Dr’s push a one time fix all only to sell their treatments. The rational people are starting to call it a chronic disease. I was given 2 choices to treat mine, radiation or prostatectomy. I see those 2 options as going to a restaurant that has 2 items on the menu, dog crap or cat crap. They’re both the same. I’m now looking into Tulsa Pro and Cyber Knife. I might make a bad choice but have no desire to just sit back and live with a lower quality of life. I can always have more something else done if needed. Nobody cares about me more than ME.     Dr’s won’t take care of your best interest as you will. I was diagnosed with prostrate cancer in Oct 2024. I’ve seen about 9 dr’s to date. I’m driving the bus even if it goes off the cliff.

      @@Tenerifelover to be honest I just take whatever jet 2 gives me as a standard package. I don't declare nothing. Thing is I don't need any treatment etc.

      @neildavidson7768 but if something happened and you needed any treatment for something the insurance will check with your GP and could refuse to pay up. Staysure wouldn't cover me until I got the all clear and now it costs me nearly £300

I just wanted to ask you I had my prostrate removed three weeks ago and I’m having some major problems with incontinence which is really getting me down. I’m not sure if you’ve done a video about this but I just need some positive advice. I just wondered how you was. ?I know everybody is different Thanks karl

      Karl, thanks very much firstly for watching. I would love to be able to answer your question but i cant!. Everybody needs to be able to look after their catheter (pain in the arse). I got a UTI which was cleared up with antibiotics. So after a month I didn't have any problems what so ever. I think everybody is different because the surgeon will have to trim back the nerves if you had an issue on 1 side. For me I had a positive margin whichIi think was close to the nerve which looks after the old man. He still hasn't recovered as yet. So I'd love to be able to help but I was lucky that I haven't got to wear pads for anytime. I really feel sorry for people like yourself. I suppose the professionals will say keep doing your kegels exercises and hopefully it will help. can I ask where abouts you are from. Again appreciate you watching mate.       Karl, in early May I had my prostate removed. After my catheter was removed, I had major issues with incontinence. I needed to wear Depends. It was quite unsettling. I started on the Kegel exercises regularly. [in fact I found an app for my smartphone to remind me to do them!]. Bottom line, it WILL get better! Now I have graduated from the Depends to a "guard" and ultimately I hope to just wear a "shield". There are some videos on incontinence after prostate surgery. Just google them. I understand completely how tough it is! Feel free to message me.

      Karl, I was a radical prostectomy, Gleason 4 + 3, PSA 16. My experience basically went: Catherer out, full depends (actually Tena, cheaper in Aust). This for maybe 2 weeks, them into thickest pads, again for a few weeks. I had what I would call a relapse in the progress. From that though, I learnt that 1: Its OK to use, or try, the pelvic floor, but like all muscles, it will tire and then not function all that well at all, Keep up the exercises but do not over do it. 2: My recovery seemed to accelerate when I started the "bladder retraining" as well as trying to not overuse the pelvic floor (I had a habit of pre-emptive stressing almost a permanent tensing which was an issue). It may have been a coincidence, but in terms of reasoning, it fits, 3: Beware the literature that gives you "indicitive times for functional recovery". I found a bit to much reliance on these initially, then confusion with the wildly varying estimates and why didn't I fit into these descriptions. 4: There is no manual or users guide on all of this. The school of hard knocks and hind sight was what eventually made sense. Out of all this, and I think Neil says it as well..... we all have INDIVIDUAL experiences through all this, its just that most experiences can sorta kinda relate to others. Ask away to others, 1st hand experience in lieu of book learning works better. Good Luck (and Happy Christmas and New Year to all). And to take heart: Statistically you WILL recover, its just a when...for most of us.

      Karl, i had my prostate removed 3 years ago. They say start the kegel exercises 4 to 6 weeks before the operation. i never did, the doctor explained when they remove the prostate some of the bladder muscles are removed or damage, but over 3 to 6 months the remaining muscles will get stronger, the rest of the recovery will be in small steps, BUT every person different when comes to incontinence, for me 3 yrs later i still a wear pad for when i goes to sleep at night as insurance since i am unconscious, for the 1st year coughing and passing gas will cause you to leak it is normal till your remaining bladder muscle get stronger. I never did pelvic floor exercises and i got over it in time so can you.

      Incontinence issues are common, mine lasted for about six months, but every month there was an improvement until finally all was under control. Use pads of course to give you more confidence and know that in almost all of the cases the problem sorts itself out with time. Indeed it can be very frustrating at times but sunnier days are ahead of you!!
